As an advanced life support technique, Extracorporeal membrane oxygenation (ECMO) plays a very important role to provide prolonged extracorporeal gas exchange and perfusion. It has been save thousands of lives with severe acute cardiopulmonary failure or congenital abnormities who were deem to die since being applied in clinic field. The survival rates of respiratory failure in newborn and children who accepted ECMO were higher than in adult, which inspired many pediatricians to use this innovative therapy. To date, the low flow rate VV-ECMO is recommended to treat patient with respiratory failure without any cardiac dysfunction for its marvelous function of removing CO2. Total compliance was applied to assess lung function during ECMO procedure and to some extent related to surfactant. It was reported that the patients' total compliance decreased just onset ECMO.
