Skin and soft tissue expansion has been an important theropy method and a basic technique in plastic and reconstructive surgery commonly used by plastic surgeons. The pre-expanded extra-long arterial fasciocutaneous flap is the extension of the pre-expanded axial pattern fasciocutaneous flap. This flap combines the techniques of tissue expansion, axial pattern flap and fasciocutaneous flap together perfectly. It has the merits that can not be exceeded by traditional techniques. But the pre-expanded extra-long arterial fasciocutaneous flap will be local necrotic sometimes after transplantation. Following with the development of tissue expansion and flap technique, how to promote the flap take for better clinical application has become an important tectonic topic.The goal of our animal studies and clinical research is to investigate the effects of tension and capsular tissue to the blood flow of the pre-expanded extra-long arterial fasciocutaneous flap. We study the changes of flap capillary hemodynamics during incremental increases in applied tension, in order to look for the best tension limit; to study the effects of capsulectomy and intact capsular tissue to flap capillary hemodynamics , oxygen metabolism and flap viability. It provides references for capsular clinical procedure.We also do some clinical research in this area.Scent pig was the model of animal studies. To design an 5cm × 20cm area on bilateral back of every pig symmetrically, implant a 300ml tissue expander. Sterile isotonic Saline was injected into expanders slowly for 2 months.
