Abstract Objective: To investigate the usefulness of ~1H MRS in glioma. Materials and Methods: We performed in vivo ~1H MRS in 33 patients who had primary intracranial gliomas and 34 patients of other intracranial diseases. Expression of Ki-67 in specimens was detected by immunohistochemical method. NAA, Cr and Crp in specimens were detected with reversed HPLC. Results: (1) Cho,NAA, Lip-Lac, NAA/Cho and Cho/Cr are of significant value in glioma glading and can differentiated with meningioma and other intracranial diseases. (2) Cho/NAA and Cho/Cr were found significantly correlated to the expression of Ki-67 respectively. (3) There was significantly correlated between ~1H MRS of gliomas and in vitro neoplastic tissue biochemical detection in NAA and Cr+PCr. Conclusion: The ease and accuracy of ~1H MRS make it a useful clinical tool in glioma glading, deciding tumor region and differentiating with other intracranial diseases. Tumor proliferative potential and prognosis can be indirectly known by calculating the ratios of Cho/NAA and Cho/Cr. ~1H MRS is a credible method that inflect intracranial metalism.
