Endometriosis(EM) is common among women with reproductive age. Its morbility is 10% among reproductive women and its incidence has been increasing. Although EM is a benign gynecological disease, its biological action is similar to malignant tumors'. People didn't know the pathogenesis really, so drug treament, surgical treatment or combination treatment are available in clinic. In late 1990s mifepristone was introduced to the medical management of endometriosis. In a series of studies, people found the good effect by using different dosage of mifepristone (50-100mg/d, 25mg/d, 12.5mg/d and 10mg/d). All patients had amenorrhea, pelvic pain improved and little side effects. Recently the effect of low-dose mifepristone (5mg /d) has been disputed, so we design this experiment. [Objective] To investigate the mechanism of mifepristone on EM and find the perfect dose . [Methods] 120 SD rats were to be induced endometriosis, 10 out of which were chosen as the control group randomly ,whose uterine were not be maken endometrium transplant. The rest underwent the surgery of endometrium transplant. There are 80 rats of 110 were successful in creating models by light microscope after 4 weeks. The 80 rats were divided into 6 groups randomly, control group: giving water 1ml daily; gestronone group: giving gestronone 0.25mg/kg, every 3 day; mifepristone groups: A group 5mg/kg daily, B group 2mg/kg daily, C group 1mg/kg daily, D group 0.5mg/kg daily. After 4 weeks, all rats were killed, and their ectopic endometrium's volume were measured by compass and the histology were observed through the light microscope and the electron microscope. The apoptosis rate and the cell cycle of each group were detected by FCM. The Fas, survivin, MMP-9 and TIMP-1 of each group were detected by FQ-PCR. [Results] ①Eutopic endometrium became larger to form a cyst with serofluid. We used the light microscope to observe the histology of ectopic endometrium to make sure that there are endometrium, glands and interstitial tissue. ②The volume of mifepristone and gestronone is atrophy and there is a significantly difference on the eutopic endometrium volume between drug group (mifepristone and gestronone) and control group(P<0.05). There is a significantly difference on the inhibition ratio between CD groups and the other groups(P<0.05). ③Light microscope show that the number of glandular cavities of ectopic endometrium decreased and the morphous of gland changed; electron microscope shows the alteration of cell junction and organelle and the appearance of apoptosis body. ④FCM shows the apoptosis of the ectopic and eutopic endometrium of each group, the apoptosis of control group decrease and significantly increase after giving drugs. There is no significantly difference on apoptosis between A and B groups and gestronone group(P >0.05). There is significantly difference on apoptosis between CD groups and AB groups(P<0.05). Except A group, there is no significantly difference between eutopic endometrium and ectopic endometrium( P>0.05 ) .The appearance of apoptosis by blocking cell cycle to S stage. ⑤The results from FQ-PCR show: low expression of fas and high expression of survivin gene of control group lead to apoptosis. On the contrary, highly-expressed fas and lowly-expressed survivin of mifepristone and gestronone depressing apoptosis as to therapy endometriosis.
