Objective The present study aims to explore the anatomical, pathogeny, pathogenesis of DAVF, and find the effective treatment method. The questions of development rule of dural mater vessel, the structure of sinus, the anastomosis of arteriovenous in dural are unknown. The main target of this study aims to explore the hemodynamics of DAVF using the methods of vessel perfusion, vessel casting and immunohistochemistry. It is hoped to obtained new treatment method of DAVF which can be validated in clinical. Method 26 fetus and 10 adults were used in this study. The vessel perfusion method, vessel casting method, anatomical skill and immunohistochemistry method were adopted in this study. The dural mater vessels and structure of sinus were observed under nude eye and the microscopy. The characteristics of clinical diagnosis and treatment of 65 DAVF was analysed.Result (l)The dural arteries begin to grow in 8 weeks fetus, the trunks of dural arteries has appeared in 12 weeks fetus but seldom branch, the dural arteries have developed entiretyly in 16 weeks fetus and insufficient branches, the dural arteries networks have formed in 20 weeks fetus and plenty of anastomosis branches, resembling the adult. (2)The dural veins develop isochronal with dural arteries, the ratio of dural vein to dural artery is 2:1. The dural sinus derived from dural venous plexus and developing later in time. The cranny of two layers in dural mater become large gradually so as to form various dural sinuses. The dural sinus is variant in developing course,there are great difference in every individuals. (3)The blood supply of dural mater in fetus is from branches of the internal carotid artery, the external artery and vertebral artery, there are plenty of anatomosis within all the arteries from three origins. (4)There are direct anastomosis branch between the dural artey and dural sinus. The distal of dural artery enter into the dural sinus. There is a plunger in fistula. (5)The dural mater is divided into three layers, the out layer and inside layer is fiberblast and fiber, the mesosphere is vessel and loosen fibers. (6)It is identified that there are smooth muscle cells in the transverse sinus in 28 weeks fetus using the SABC immunochemistry method. (7)The density of dural arteies is different in sinus and non-sinus area, the density of dural arteies in sinus is 2 to 4 times than that in non-sinus area. There are plenty of anatomosises within all the arteries from different origins. (8)The dural sinus in adult is varition. The boundary of transverse sinus and sigmoid sins become straitness and it is easy to form thrombus. There are plenty of communication branches between each dural sinus. Transverse sinus and sigmoid sins connect with extracranial vien by route of many communication veins. Posterir fossa vein communicate with the spinal medullary vein. (9)The dural sinus exist Cavernous body resembling penis structure. Sponge can make the sinus cavity shrinking or expanding in order to adjust the cerebral blood flow via nerve reflex. (10)there isn't direct anastomosis between the dural artery with dural veins and dural sinuses. (ll)The dural arteries drain into dural sinus by two means. One way is that dural artery enter into arachnoid graulations, another is that dural artery penetrating into the sinus wall and finally inflow to sinus. This is potential anastomosis. (12)The diameter of dural arteries which locate in dural sinus wall is 50-300 u m. These arteries may become feeding arteries of fistulas. (13)The venous pool in dural mater existence because dura bad healing up, and communicating with sinus. (14)The feeding arteries to DAVF drain to one sinus, but DAVF can drain to cortical veins and pia mater veins. (15)DSA is the best method to diagnose DAVF. DAVFs often occurred in cavernous area. The treatment methods include observation, operation, embolization andradiology method.
