Nonarteritic anterior ischemic optic neuropathy (NAION) is a common ischemic optic nerve disease with serious damage to visual function of old people, and usually attributed to the disease category of "Blurred vision"or "Sudden loss of vision" in Traditional Chinese Medicine (TCM) based on the etiology and pathogenesis. Up to now, the pathogeny is undefined and there is no generally accepted effective treatment.Good therapeutic effect were often reported in most TCM documents and clinical experience summaries on the basis of symptoms and signs differentiation. But researches on the syndrome classifications and treatments of NAION are based on the clinical experience or the simple statistical method, so the conclusions are unscientific and untypical. Up to now, the treatment of TCM for NAION is short of standard which limits the advantage of TCM for NAION. During the system of syndrome differentiation and treatment, syndrome is the center of theory, method, prescription and medicine. Because of these, professor Jian Zhouconsiders it is imperative to carry out standardization study of NAION in the aspect of TCM syndrome study now. This research is using multivariate statistics method like cluster analysis, factor analysis on the basis of cross-section survey to investigate the syndrome type of NAION, in order to lay the foundation for the prospective standardized clinical research. The thesis consists of the literature review and clinical observation. The literature review includes two parts, the research of western medicine and TCM of nonarteritic anterior ischemic optic neuropathy (NAION).In the first part, it mainliy records the definition, classification, etiology, clinical features, auxiliary examination and treatment of NAION. In the second part, it mainly review the medical name of disease and historic evolution, the disease certificate back, the etiology and pathogenesis, the syndrome differentiation and treatment.Clinical observation part:Purpose:This paper summarizes the domestic and overseas study progress for NAION using TCM and western medicine. It mainly discusses the syndrome type of NAION using multivariate statistics method. Methods:A prospective cross-section investigation is carried out. Firstly, we make a TCM syndrome questionnaire of NAION through expert consultation and liter-ature review. Then clinical information of 254 NAION patients from the ophthalmology of Dong Fang Hospital, The third affiliated hospital of Beijing University of Chinese medicine along with Traditional Chinese Ophthalmology Hospital were surveyed and collected using the TCM NAION questionnaire during the period of the January of 2014 to December of 2015.All of those clinic study documents were put into the Epidata and EXCEL database. At last, the main types of NAION symptom-complex classification is summed up using the multivariate statistics method.Result:â‘  In this investigation study, the former ten listed symptoms of the 254 NAION patients is diminution of vision, dark shade of eye, fullness in chest, dizziness, bitter taste, thirst, irritability, chest stuff, palpitation and depression. The often color of tongue nature is pale, dark red with ecchymosis, dark and red. The frequent tongue coat is thin white, scanty fur and white fur. The frequent pulse is string stringer, stringy pulse, rapid pulse and sluggish pulse.â‘¡ the 50 mainly symptoms of 254 cases are clustered into four clusters:blood stasis due to qi deficiency type, stagnation of liver and qi stasis type, yin deficiency with yang hyperactivity type, qi stagnation with blood stasis type. â‘¢ Through principal component analysis,9 main constituents are concluded and merged as:fire flaming from yin deficiency type, blood stasis due to qi deficiency type, stagnation of liver and qi stasis type, qi stagnation with blood stasis type. â‘£Through factor analysis.4 kinds of syndrome are concluded as:fire flaming from yin deficiency type, blood stasis due to qi deficiency type, stagnation of liver and qi stasis type, qi stagnation with blood stasis type. Conclusions:Blood stasis syndrome, qi stagnation syndrome, qi deficiency syndrome, stagnation of liver and yin deficiency syndrome are frequently appeared in NAION patients. And fire flaming from yin deficiency type, blood stasis due to qi deficiency type, stagnation of liver and qi stasis type and qi stagnation with blood stasis type are frequently seen in NAION clinical practice.
