Purpose:Objective To investigate the effect of stage dressing change after anal carbuncle of fire poison incandescent syndrome.Method:Sixty patients with anal carbuncle(fire poison incandescent syndrome). The patienta who were admitted to the anorectal department were randomly divided into two groups:the routine dressing change group and the stage dressing change group,30 cases in each group, and received one-time incision and suture treatment. The conventional dressing change group was treated with one-effect paste and Vaseline yarn, and dressing change twice a day.Stage dressing change group was divided into 1 to 3 days after surgery, using vaseline gauze; On the 4th to 7th day after operation,Vaseline yarn strip plus an effect paste was applied;8-14 days after surgery,Vaseline yarn strips plus one-effect paste plus elephant skin powder were applied, dressing change twice a day. After 14 days of treatment, postoperative anal pain, anal border skin edema, wound secretion, wound granulation tissue growth and wound area healing rate of patients with fiery fiery syndrome anal carbuncles were compared, and then the overall curative effect was observed. This thesis has passed the Ethics Committee of Affiliated Hospital of Traditional Chinese Medicine of Liaoning University.(Ethics Approval No.: 2021068FS(KT)-045-02)Result:1.There was no statistical difference in postoperative and on the day of operation and on the 3rd day after operation(p>0.05),and pain scores on the 7th and 14 th postoperative days were lower in the staggered dressing exchange group than in the conventional dressing exchange group, indicating that there was a statistical difference between the two groups(p<0.05).2.For skin edema at the anal verge, the values of the staggered dressing exchange group were better than those of the conventional dressing exchange group on postoperative days 3 and 7,and statistically significant differences were found(p<0.05),but at the 14 th day after surgery, the value of the two groups were similar, with no satistical significance(p>0.05).3.The results indicated that there was no significant difference in granulation growth on the3 rd day after operation(p>0.05).Statistical significance was found between the two groups(p< 0.05).4.In the stepwise postoperative drug exchange group, the secretion values were higher on postoperative days 3 and 7 compared with the conventional drug exchange group, and statistically significant differences were observed in both groups(p < 0.05).At day14,there were no significant differences and no obvious satistical significant(p>0.05).5.The study showed that the healing time was significantly shorter in the staged dressing change group and there was a statistically significant difference compared to the conventional dressing change group(p < 0.05).6.The results indicated that there was no statistical difference on the third and seventh day of the operation in the wound healing rate.showed that in terms of wound healing rate(p>0.05),while the wound healing rate was significantly faster in the staged dressing change group on day 14 compared to the conventional dressing change group, with a statistically significant difference between the two groups(p < 0.05).7.The study showed that after 14 days of treatment, the healing rate was significantly higher in the staged dressing change group compared to the conventional dressing change group(p<0.05).8.The study showed that postoperative WBC count,C-reactive protein and fibrinogen were significantly lower in both the incremental change group and conventional change group compared to the preoperative group, and statistically significant differenceswere found(p<0.05),but there were no statistically significant differences(p>0.05).Conclusion:1.Both the conventional dressing change group and the phased dressing change group are effective in promoting postoperative wound healing of patients with anal carbuncle with excessive fire and poison syndrome.2.The staged dressing change group is superior to the conventional dressing change group in pain, granulation growth, anal edema, wound secretion, healing time and 14-day healing rate.It is suggested that dressing change by stages can promote healing, smooth drainage, eliminate edema and relieve postoperative wound pain in patients with anal carbuncle with excessive fire and toxin syndrome.
