Professor Wang Xingkuan's Experience In Treating Insomnia And A Preliminary Exploration Of The Mechanism Of Action Of Core Prescriptions And Medicine

Objective: To conduct data mining on Professor Wang Xingkuan’s medical case of treating insomnia,and to explore the medication rule of treating insomnia.In addition,network pharmacology and molecular docking study of the obtained core prescriptions was conducted to explore their mechanism of action,explain their scientific nature,summarize Professor Wang Xingkuan’s academic thoughts,and guide clinical practice.Method:(1)Medical records of Professor Wang Xingkuan diagnosed as insomnia from January2018 to December 2020 were collected and used Microsoft office excel 2016 sorted out and entered the original medical record data,established a database,and conducted frequency analysis,cluster analysis and association rule analysis through the TCM inheritance assistance platform(V2.5)system.(2)The core prescriptions of Professor Wang Xingkuan for insomnia were selected for network pharmacology and molecular docking research.TCMSP database,ETCM database and BATMAN-TCM database were used to screen the active ingredients and related targets of the core prescriptions.The related targets of insomnia were obtained from Gene Cards database,OMIM database and TTD database,and drug-disease intersection targets were obtained and drug-compotion-target network map was constructed.Protein-protein interaction(PPI)network was constructed based on STRING website and core target screening was conducted.GO enrichment analysis and KEGG pathway enrichment analysis were performed for drug-disease intersection targets using R-3.6.3 software.Finally,the Sail Vina software is used to perform molecular docking of core targets and key components and analyze the results.Results:(1)Professor Wang Xingkuan’s distinguished treatment of insomnia is the most commonly observed type of liver and gallbladder loss,mind loss,phlegm-heat internal disturbance,Qi and Yin deficiency,cardiopulmonary dysnourishment,heart and kidney disharmony,mind loss,liver-yang hyperactivity,liver and kidney deficiency,the treatment is based on stability,liver and gallbladder,clearing heat and phlegm,qi and Yin,communication of heart and kidney,tonifying liver and kidney.The core prescriptions include fried sour jujube kernel,Fu God,Bupleurum,Scutellaria,scorched licorice,lily,Fructus aurantii,Zhizhi,albizia bark,bamboo shavings.It is consistent with Professor Wang Xingkuan’s theory of "the heart hides the God,the lung hides the soul and the liver hides the soul" when he distinguished insomnia,and he proposed the academic thought of the three-dimensional system of "the heart,the liver and the lung-the soul of the God".(2)Network Pharmacology and molecular docking studies: After screening,a total of 240 active compounds in the core prescriptions were obtained,including 9 in sour jujube kernel,17 in bupleurum,36 in scutellaria scutellaria,92 in licorice,7in lily,22 in Fructus aurantii,25 in Radix auricularis,25 in Radix auricularis bark,3 in bamboo Shau and 4 in Fu Shen.The key active compounds include quercetin,carotene,kaempferol,stigmasterol,naringin,isorhamnetin,β-sitosterol,luteolin,baicalin,etc.GO functional analysis obtained 2961 items,including 2571 biological process(BP)items,166 cell composition(CC)items and 224 molecular function(MF)items.KEGG analyzed 184 related pathways.The results of molecular docking showed that the binding energy between the key ingredients and the core target was less than-7.0 kcal/mol,and there was good binding activity between the active ingredients in the core prescriptions and the core target for treating insomnia.Conclusion:(1)Professor Wang Xingkuan argued that "the heart hides the God,the lung hides the soul,and the liver hides the soul" in distinguishing insomnia,and put forward the three-dimensional system of "the heart,the liver and the lung-the soul of the God","How viscera tune Xie".Partial solid people with cardiopulmonary dysnourishment,liver and gallbladder loss,phlegm heat internal disturbance,god’s soul is difficult to enter the house as the basic pathogenesis,nourishing the heart and lung,Qi Yin,soothing liver wood,phlegm heat,calming the spirit and calming the soul as the basic treatment.(2)Professor Wang Xingkuang’s core prescription for treating insomnia has the characteristics of multi-component,multi-target and multi-pathway,and its mechanism of action may be the active ingredients contained in quercetin,carotene,kaempferol,soy sterol,naringin,isorhamnetin,β-sitosterol,luteolin,and baicalin.It acts on MAPK1,MAPK3,STAT3,HIF1 A,IL6,TP53 and other targets,and exerts therapeutic effects on insomnia through neural active ligand-receptor interaction pathway and MAPK signaling pathway.
