Purpose: Data mining technology was used to explore the drug use rules of traditional Chinese medicine in the treatment of obtain core prescriptions,chronic renal heart syndrome,and guide the clinical medication.With regard to core prescription,the potential mechanism of action of core prescription in treating chronic renal heart syndrome was explored by using the research method of network pharmacology.Materials and Methods:1.Patients admitted to the Department of nephrology and Cardiology of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from September 2019 to September 2022 were included.,who met the diagnostic conditions for chronic renal heart syndrome and received oral treatment with Chinese medicine decoction.The data were standardized processed and a database was established,and the drug frequency,sexual and taste normalization and efficacy were statistically analyzed.Using SPSS Modeler 18.0 software for drug association rule analysis,SPSS Statistics 25.0 software was used for cluster analysis,and medication rules and core prescriptions of CRCS were obtained.2.Conduct network pharmacological analysis of core prescriptions,and use TCMSP database to screen the active ingredients and action targets of drugs in core prescriptions.For ophiopogonis ophiopogonis and Radix glycyrrhizae,which are not queried in TCMSP database,query relevant literature to collect their active ingredients,and use Pub Chem database to obtain the chemical structure of active ingredients.Then import the data to the Swiss ADME data base for screening,then import the structure of the active pharmaceutical ingredients to Swiss Target Prediction data base to gain the active ingredient and action target.Then we use Uniprot database to find out the drug corresponding target gene.Take the intersection of the drug and the disease to gain the drug-disease interaction.Update the interaction to the String database to construct the PPI network.Analyse the GO function and KEGG pathway with DAVID,visualize the data by Weishengxin website,construct the drug-ingredients-target network by software Cytoscape 3.9.1,predict and discuss about the principal of core prescription.Results:1.This research included 116 patients with CRCS in total.A total of 254 prescriptions were collected,involving 242 kinds of drugs.Drugs with sweet,bitter and xin were used most commonly.Drugs that benefits the spleen,liver,lung,heart and kidney have been used.Among the drug efficacy,the most commonly used drugs were tonifying deficiency was the most used drug,which promots blood circulation and removes blood stasis,and enhancing water and dampness.According to the drug association rules,the top five drug combinations were: Perilan-Atractylodes,pseudostellariae-atractylodes,cuscuta-Tangerine peel,cuscuta-atractylodes and sarcos-Poria.Three kinds of prescriptions were obtained by cluster analysis.The core prescriptions were: poria cocos,white Rhizoma,prepared licorice,angelica sinensis,Astragalus,Tangerine peel,white peony root,salvia miltiorrhiza,cassia twig,Radix ophiopogonis,ligusticum Chuanxiong.2.A total of 197 active ingredients,621 target proteins,9920 disease-related targets were obtained,186 drug-disease joint targets were screened,and 185 kinds of corresponding active ingredients were detected.The main active ingredients included luteolin,quercetin,luscogenin,kaempferol,etc.,in the core prescription.The main targets were STAT3,AKT1,HSP90AA1,SRC,EGFR,TP53,JUN,IL6,VEGFA,TNF,etc.Biological processes mainly involve the regulation of gene expression,the response to drugs,the positive regulation of cell proliferation and migration,and the response to hormones.In cell composition,receptor complex,extracellular space,membrane raft,plasma membrane and so on are mainly affected.The molecular functions mainly involve the activity of transmembrane receptor protein tyrosine kinase,enzyme binding,protein binding,ATP binding,protein homopdimer activity,transcription factor binding,etc.Enrichment analysis of KEGG pathway included AGE-RAGE signaling pathway,PI3K-Akt signaling pathway,HIF-1 signaling pathway and MAPK signaling pathway in diabetes complications.Conclusion:1.The treatment of this disease when the drug is based on warm drugs,pharmaceutical flavor for sweet bitter xin,the treatment of multiple qi and blood double complement to cure the root,activating blood stasis,water infiltration to treat its standard.Drugs attributed to the spleen,liver,lung,heart,kidney channels have been used.Commonly used drugs include poria cocos,white Rhizoma,Astragalus,sunburned licorice,Danshen,angelica,tangerine peel,white peony,Radix pseudostellariae,cassia twig.The core prescriptions were: poria cocos,white Rhizoma,prepared licorice,angelica sinensis,Astragalus,Tangerine peel,white peony root,salvia miltiorrhiza,cassia twig,Radix ophiopogonicus,ligusticum Chuanxiong.Clinical medication needs to vary from person to person,and increase or decrease with the syndrome.2.The core prescription contains a large number of active ingredients such as luteolin,quercetin,Luscogenin and kaempferol,which play a role in the treatment of CRCS by affecting targets such as STAT3,AKT1,HSP90AA1,SRC,EGFR,TP53,JUN,IL6,VEGFA and TNF.CRCS are treated by reducing inflammatory response,antioxidant stress and other reactions,and affecting AGE-RAGE signaling pathway,PI3K-Akt signaling pathway,HIF-1signaling pathway,MAPK signaling pathway and other related pathways in diabetes complications.An active ingredient in the core prescription can regulate multiple targets,and the same target can mediate multiple signaling pathways,reflecting the characteristics of "multi-target and multi-pathway" in traditional Chinese medicine. |
