| Purpose:To observe the difference of clinical effect between dydrogesterone tablet combined with Shugan Jianpi formula and dydrogesterone tablet alone in treating patients with abnormal uterine bleeding caused by polycystic ovary syndrome(PCOS),analyze the quantitative score of TCM symptoms and the related indexes of sex hormones in the two groups of patients under each scheme,to explore the therapeutic effect of Shugan Jianpi formula combined with dydrogesterone tablet on abnormal uterine bleeding caused by polycystic ovary syndrome(PCOS),and to provide the therapeutic idea and experimental data for clinic.Material and method:Between September 2021 and November 2022,a total of 66 patients with abnormal uterine bleeding(syndrome of stagnation of the liver and deficiency of the spleen)caused by PCOS were selected as the study object from the gynecological clinic of the Affiliated Hospital of the Liaoning University of Traditional Chinese Medicine,use random number table method,all patients were divided into two groups(the observation group and the control group),there were 33 cases per group.The patients in the control group were treated with dydrogesterone tablet,the patients in the observation group was treated with Shugan Jianpi formula and dydrogesterone tablet.The TCM syndrome score,serum sex hormone level,ultrasound condition,hemostasis time and menstrual effect were compared between the two groups,and comprehensive evaluation of the efficacy.Results:1.Comparison of general conditions:There was no significant difference in age,disease course and BMI between the two groups(P>0.05),the general data of the two groups were comparable.2.Comparison of TCM syndrome integral:After treatment,the total scores of TCM syndromes in the two groups were significantly different from those before treatment(P<0.05),after treatment,the comparison of the total integral between the observation group and the control group was statistically significant(p<0.05).3.Comparison of short-term hemostatic efficacy:In terms of hemostatic effect,The total effective rate was 93.33% in the observation group and 63.33% in the control group.The comparison between the two groups was statistically significant(P=0.010<0.05).4.Adjust menstruation curative effect contrast:In the observation group,21 cases were cured,7 cases were markedly effective and 2 cases were effective.In the control group,18 cases were cured,6 cases were markedly effective and 6 cases were effective.There was no significant difference between the two groups in the curative effect of regulating menstruation(p=0.640>0.05).5.Sex hormone index comparison:After treatment,the LH,T,LH/FSH values in both groups were significantly lower than those before treatment,there was significant difference between the two groups.After treatment,the levels of LH,LH/FSH in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference in FSH and T between the two groups after treatment(P>0.05).6.Comparison of anxiety and depression scores:After treatment,the scores of depression and anxiety scale were significantly decreased in both groups(P<0.05),the scores of depression and anxiety scale in the observation group were significantly lower than those in the control group(P<0.05).7.Follow-up record:During the follow-up,the recurrent rate of abnormal uterine bleeding was10%(3 cases)in the observation group and 36.67%(11 cases)in the control group.There was significant difference between the two groups(P=0.003<0.05)The difference of menstruation between the observation group and the control group was significant(p=0.000<0.05),the long-term effect of menstruation regulation in the observation group was significantly better than that in the control group.Conclusion:1.The therapeutic scheme of Shugan Jianpi formula combined with dydrogesterone tablet is more effective than that of dydrogesterone tablet alone in the treatment of abnormal uterine bleeding caused by PCOS.2.The treatment of abnormal uterine bleeding caused by polycystic ovary syndrome(syndrome of stagnation of the liver and deficiency of the spleen)with modified Shugan Jianpi formula combined with dydrogesterone tablet can effectively improve the clinical symptoms of the patients,adjust menstruation and serum sex hormone level of the patients,improve the depression and anxiety state of patients,no obvious adverse reactions were observed.3.Shugan Jianpi formula can effectively prevent recurrence of abnormal uterine bleeding,safe and reliable,worthy of clinical promotion. |
