Purpose:In this paper,the distribution and combination of TCM syndrome elements in type 2 diabetes mellitus patients with mild cognitive impairment and its clinical influencing factors were studied in young and middle-aged people,in order to provide objective theoretical basis for clinical diagnosis and treatment.Material and method:According to the expert consultation report of WHO and 1999 of the international diabetes federation,232 young and middle-aged inpatients with type 2diabetes mellitus aged 35-60 years were randomly selected.They were divided into two groups: 133 patients with cognitive impairment of type 2 diabetes mellitus and 99 patients with non-cognitive impairment of type 2 diabetes mellitus.According to the contents of the four diagnostic tests in the textbook diagnostics of Traditional Chinese Medicine(TCM),and referring to the part of syndrome of TCM clinical diagnostic and treatment terms of the national standard of the People’s Republic of China,the data of the four diagnostic tests of patients were collected,according to the steps and methods of the international scale,a diagnostic scale of TCM syndrome elements was developed to analyze the distribution of TCM syndrome elements in type 2 diabetes mellitus patients with MCI.Results:1 There were 223 syndrome types in 133 cases of type 2 diabetes mellitus with MCI,including 90 syndrome types of phlegm-dampness(67.6%),52 syndrome types of spleen deficiency(39.1%),123 syndrome types of phlegm-dampness(67.6%),133 syndrome types of type 2 diabetes mellitus with MCI(39.1%),133 syndrome types of phlegm-dampness(67.6%),133 syndrome types of spleen deficiency(39.1%)As for the combination of TCM syndrome elements,the most common one was spleen deficiency syndrome and phlegmturbid syndrome,which were combined 49 times(36.8%),followed by phlegm-turbid syndrome alone(28 times),the percentage was 21%,which indicated that spleen deficiency was closely related to phlegm.2 There were significant differences in age,course of disease,education,BMI,smoking history,drinking history between the two groups(p < 0.05),the chi-square value of peripheral neuropathy change in peripheral vascular disease was significantly higher than that in control group(p < 0.05).The levels of Hb A1 c,CHOL and LDL-C were significantly higher than that in control group(p < 0.05).Conclusion:1.There is a close relationship between spleen deficiency and phlegm-turbidity,which may be the core pathogenesis of cognitive dysfunction in type 2 diabetes mellitus.2.Age,course of disease,education,smoking,drinking,BMI,complications,Hb A1 c,CHOL and LDL-C were the important influencing factors of cognitive impairment in type 2 diabetes mellitus. |
