Related Factors Of Cognitive Impairment In Type 2 Diabetes Mellitus Patients

[Objective]The aim of this study was to explore the risk factors of cognitive dysfunction in type 2 diabetes mellitus(T2DM),and to provide the clinical basis for the prevention and treatment of cognitive dysfunction.[Methods)A total of 96 patients with T2DM who were treated in our hospital were enrolled in the study.The Montreal Cognitive Assessment(MoCA)was used to measure the cognitive function of T2DM patients.According to MoCA scores,96 T2DM patients were enrolled were divided into cognitive dysfunction group and cognitive function of normal group.By analyzing the clinical data of the two groups of patients and combining the univariate and multivariate unconditional logistic regression analysis,the multiple linear regression analysis of T2DM patients in the cognitive dysfunction group was carried out to clarify the related factors of T2DM patients’ cognition dysfunction.[Results]The mean age of patients with cognitive dysftunction(n=46)was 58.5 ±6.2 years,years of education was 6.84 ± 2.14,duration of disease was 8.80±2.04 years,HbAlC was 10.17 ± 0.97%,total cholesterol(TC)concentration was 5.02 ± 0.66mmol/L,the uric acid(UA)concentration was 372.8±49.7mmol/L,insulin Ins(0.5h)was 44.76±14.46 and insulin Ins(2h)was 60.65 ± 14.13.The difference was significant(p<0.05)when compared with the normal cognitive function group(n=50)by t-test.The scores of MoCA scale in patients with T2DM cognitive dysfunction were significantly lower than those in normal cognitive function group,including visual space and executive function,naming,attention,language,abstraction,delayed recall,orientation.There was significant difference between the two groups(P<0.05).The correlation analysis between cognitive impairment and other related factors in T2DM patients showed that exercise and education were the positive factors to ensure normal cognitive function.There were significant correlations between age,duration of disease,glycosylated hemoglobin level,lipid concentration,uric acid concentration,insulin concentration,diabetic retinopathy,diabetic nephropathy,diabetic peripheral neuropathy and atherosclerosis and cognitive dysfunction.Results of unconditional logistic regression analysis showed that age,glycosylated hemoglobin level,uric acid concentration,and insulin level were the independent risk factors of cognitive dysfunction in T2DM,while exercise was the protective factor.[Conclusion]Age,glycosylated hemoglobin,uric acid,glucose tolerance test 2 hours insulin level is independent risk factors of T2DM cognitive dysfunction,while exercise is a protective factor.
