Objective: To investigate the effect of Yiqi Dingxuan decoction on NLRP3 inflammasome and the expression of IL-18,IL-1β inflammatory factors in diabetic rats with cerebral ischemia.Methods: 130 male SD rats were used to establish the animal model of diabetes mellitus combined with cerebral ischemia,the rats were randomly divided into Sham Operation Group,Model Group,Yiqi Dingxuan decoction group,Buphthalide Soft Capsule Group,Yiqi Dingxuan decoction combined with Buphthalide soft capsule group.After 72 hours of intervention with distilled water and corresponding drugs,the serum and ischemic brain tissue of rats were collected.The changes of neural function were evaluated by neurological function scoring,the H&E stain of brain tissue was observed,and the levels of serum inflammatory factors IL-18 and IL-1β were measured by enzyme-linked immunosorbent assay QPCR and WB were used to detect the expression of NLRP3 inflammasome and Caspase-1 protein.Results: Compared with the blank group,the brain structure of the model group was changed,and the neural function score,the content of IL-18,IL-1β,the expression of NLRP3 m RNA,Caspase-1 m RNA and corresponding protein were increased in the model group(P<0.05),the expression of NLRP3 m RNA,Caspase-1 m RNA and the expression of IL-1β,IL-18,NLRP3 m RNA and Caspase-1 protein were significantly decreased in the treatment group as compared with those in the control group(P<0.05),the levels of IL-18,IL-1β,NLRP3 m RNA,Caspase-1 m RNA and the expression of related proteins were decreased in the combined treatment group(P<0.05).Conclusions: 1.Yiqi Dingxuan decoction can improve the neurological function defect of cerebral ischemia combined with diabetes mellitus rats and exert the protective effect of neurological function.2.Compared with the model group,the number of damaged cells in the ischemic area of rats decreased after 72 hours of intervention,indicating that Yiqi Dingxuan decoction has protective effect on brain tissue.3.After 24 h and 72 h,the levels of IL-18 and IL-1β in the serum of the Yiqi Dingxuan decoction decreased,the m RNA expression of NLRP3 and Caspase-1decreased,and the protein expression of NLRP3 and Caspase-1 decreased,it is suggested that Yiqi Dingxuan decoction can inhibit the inflammatory reaction of brain cells and alleviate the damage of brain tissue by regulating the expression of NLRP3 pathway and downstream inflammatory factors.4.The combined treatment group(Yiqi Dingxuan decoction and butylphthalide soft capsule)could significantly improve the neurological deficit symptoms of cerebral ischemia combined with diabetes compared with the single Yiqi Dingxuan decoction or butylphthalide group,inhibition of NLRP3 pathway and its downstream inflammatory factors,reduce the inflammatory response of nerve cells.It is suggested that multi-pathway,multi-target and multiefficacy combination therapy can improve the curative effect of diabetes complicated with cerebral ischemia.
