Research On Professor Li Ping's Experience In Diagnosis And Treatment Of Atrial Fibrillation Based On Data Mining Technolog

ObjectiveBy using data mining,the clinical studies related to the treatment of atrial fibrillation with traditional Chinese medicine in recent ten years were analyzed,so as to gather the basic state of these studies,and summarize the characteristics of syndromes and the general prescription regularity of traditional Chinese medicine in the treatment of atrial fibrillation.By analyzing the outpatient medical records of atrial fibrillation treated by Professor Li Ping and comparing these information with general regularity,the characteristics of Professor Li Ping’s experience of diagnosis and treatment of atrial fibrillation were summarized.MethodsThe randomized controlled trials of atrial fibrillation treated by traditional Chinese medicine treatment in the past 10 years was retrieved from Chinese databases.The medical records of Professor Li Ping’s outpatient of atrial fibrillation from January 2013 to November 2022 were collected from the information system of the Third Affiliated Hospital of Beijing University of Chinese Medicine.The trials and medical records were further screened according to inclusion and exclusion criteria.In order to understand the general situation of these clinical trials,the sample size,course of treatment,disease complications,treatment plans and observation indicators were analyzed by frequency statistics.The frequency analysis of syndrome types,property and flavor,meridian distribution,pharmacologic actionand high-frequency drugs in trials’ data and Professor Li Ping’s outpatient medical records was conducted respectively.The association rules and cluster analysis were conducted for high-frequency drugs,and the similarities and differences were compared.The general rules of discrimination and treatment of atrial fibrillation by traditional Chinese medicine and Professor Li Ping’s characteristic experiences were summarized.Through complex network analysis of Professor Li Ping’s prescription for the treatment of atrial fibrillation,the core prescriptions of Professor Li Ping’s treatment of atrial fibrillation were summarized.Results1.1225 trials were retrieved,and 105 trials were finally included after screening;The outpatientl records of Professor Li Ping were screened and finally included 95 patients and 184 records.2.Most of the included studies did not specify the comorbidities.Some studies specified that the comorbidities were atrial fibrillation complicated with coronary heart disease,heart failure,hypertension,etc.In the control groups,antiarrhythmic drugs were mostly used(48.59%);The experimental groups were treated by traditional Chinese medicine on the basis of the control groups,and the self-designed empirical formulas were mostly used(67.62%).Among the observation indexes,laboratory examination(23.41%),clinical symptom effect(17.68%),rhythm control effect(15.76%),echocardiography index(13.54%)and rate control effect(10.51%)were more widely used.3.The common rules of the treatment of these trials and Professor Li Ping’s outpatient records mining are as follows:Qi deficiency,Yin deficiency and blood stasis are more common;the drug flavors are mainly warm,cold,peaceful and sweet,bitter and xin;drugs are more common in the channels of the heart,liver,lung,spleen,stomach and kidney;specific drugs are used to enhance Qi and Yin,promote blood circulation and remove blood stasis.4.Professor Li Ping’s characteristic experience of syndrome differentiation of atrial fibrillation is tongue diagnosis,emphasizing the role of phlegm-heat and wind-pathogenic factors in the occurrence and development of atrial fibrillation.In addition to the treatment of Qi and Yin,blood circulation and blood stasis,Professor Li Ping also mainly uses heat and phlegm clearing,wind and collateral clearing,and insects are widely used.The complex network analysis also summarized the core prescriptions of Professor Li Ping for the treatment of atrial fibrillation,in which the core prescriptions of Qi and Yin deficiency syndrome are composed of radix pseudostellariae,ophiopogon ophiopogon,schisandra chinensis,flavin,scorpion,centipede,leech,keel and panax notoginseng.The core prescription drugs of phlegm-heat internal distraction syndrome are:bamboo shavings,acorus calamus,tangerine peel,fructus aurantii,turniflora,pinellia pinellia,coptidis,poria cocos and radix aconitum.ConclusionCompared with Western medicine,the combination of traditional Chinese medicine and Western medicine in the treatment of atrial fibrillation may have advantages in rhythm control and rate control,and relieve the clinical symptoms and improve quality of life of patients,and has a higher safety profile than antiarrhythmic drugs.The efficacy in improving cardiac function and myocardial remodeling is not clear,and there are problems such as small sample size,short observation,and lack of long-term observation indicators.Traditional Chinese medicine’s treatments of atrial fibrillation are mainly invigorating Qi and nourishing Yin,activating blood and removing stasis,and the classic prescriptions can be used as references in the treatment of atrial fibrillation in modern times.Professor Li Ping pays attention to phlegm,heat,moving wind in diagnose of atrial fibrillation except Qi and Yin deficiency,blood stasis.The patient’s tongue can also provide references in diagnose of atrial fibrillation.In the treatment,clearing heat and dissolving phlegm,invigorating Qi and nourishing Yin are widely used,more refreshing and tonics,less warmth and dryness are used.Professor Li Ping also attach importance to the smooth regulation of the spleen and stomach.Activating blood circulation and removing stasis,extinguishing wind and dredging meridian are used throughout the treatment.
