| Objective:To select different thicknesses of milli needles for the treatment of patients with subthreshold depression of liver-depression and spleen-deficiency type,and to observe the differences in the efficacy of the two needles,so as to provide a theoretical basis for the selection of needles for acupuncture treatment of this disease.Methods:A clinical randomized controlled trial was conducted in which 60 subjects with subthreshold depression of liver-depression and spleen-deficiency type were recruited from December 2021 to December 2022 at the Department of Rehabilitation of the Third Affiliated Hospital of Beijing University of Chinese Medicine as study subjects.1.Needles selected for both groups:New Xinglin brand acupuncture needles were used,and stainless steel milli needles with a diameter of 0.20 mm(0.20 mm×25 mm;0.20 mm×40 mm)were used for the fine needle group,and stainless steel milli needles with a diameter of 0.35 mm(0.35 mm×25 mm;0.35 mm×40 mm)were used for the coarse needle group.2.acupuncture points for both groups:Yin Tang,Baihui,Shuigou,Tai Chong(double),Neiguan(double),Shen Men(double),Tan zhong,Qi Men(double),and Foot San Li(double).The acupoints were positioned with reference to the National Standard of the people’s Republic of China(GB-T123462006)"Name and positioning of Acupoints".3.Acupuncture operations in both groups:The patient was placed in a supine position,and after routine disinfection with 75%alcohol,Shuigou was stabbed obliquely 0.3-0.5 inch upward,Yintang was stabbed flatly 0.3-0.5 inch,Baihui was stabbed flatly 0.5-0.8 inch,Taichong was stabbed straightly 0.50.8 inch,Neiguan was stabbed straightly 0.5-1 inch,Shenmen was stabbed straightly 0.30.5 inch,Tanzhong was stabbed flatly 0.3-0.5 inch,Zhimen was stabbed obliquely 0.5-0.8 inch,and Foot Sanli was stabbed straightly 1~2 After obtaining the qi,retain the needle for 30 minutes,twist and turn the needle once at an interval of 10 minutes,with flat tonic and flat diarrhea.4.4.Frequency of treatment:Both groups were treated 3 times a week,once every other day,and 4 weeks was a course of treatment.After 4 weeks of continuous treatment,the therapeutic efficacy was observed.5.Therapeutic efficacy evaluation and indexes:The differences in efficacy between and within the two groups were observed using the Hamilton Depression Inventory 17 items(HAMD-17),Depression Scale(CES-D),Depression Self-Rating Scale(SDS),and Chinese Medicine Evidence Rating Scale before treatment and after 4 weeks of treatment,respectively,and the adverse effects during the experiment were recorded.Results:1.general baselineThere were 60 cases of subthreshold depression of liver-depression and spleendeficiency type that met the inclusion and exclusion criteria,30 cases in the experimental group and 30 cases in the control group,and there was no significant difference between the two groups of subjects in terms of gender,age,and education compared(P>0.05),which were comparable.2.HAMD-17 scaleThe two groups of SD patients were not statistically significant before treatment(P>0.05)and were comparable.After 4 weeks of treatment,the total effective rate of the test group was 90.00%and the total effective rate of the control group was 83.33%,Z=-3.02,P<0.05,which was statistically significant,according to the analysis of the reduction rate of the two groups of patients with the efficacy grade of the two groups,using the nonparametric rank sum test.The intra-group comparison of the HAMD-17 scale scores between the test and control groups before and after 4 weeks of treatment was statistically significant(P<0.05).After 4 weeks of treatment,the HAMD-17 scale scores of the two groups were statistically significant(P<0.05),and after 4 weeks of treatment,the HAMD-17 scale scores of the test group were significantly lower than those of the control group.The results indicated that both the test group and the control group had therapeutic effects on SD,and the treatment effect of the test group was better than that of the control group.3.CES-D scaleThe CES-D scale was not statistically significant in the two groups of SD(Subthreshold Depression)patients before treatment(P>0.05),which was comparable,and the CES-D scale scores were statistically significant in the test and control groups before treatment and after 4 weeks of treatment(P<0.05).The between-group comparison of CES-D scale scores between the two groups before and after 4 weeks of treatment was not statistically significant(P=0.856>0.05),and the results indicate that there was no difference in the treatment of subthreshold depressed subjects between the two groups with the selected needle device,and the two groups had comparable treatment effects.4.SDS scaleThe two groups of SD patients were not statistically significant before treatment(P>0.05)and were comparable.The intra-group comparison of SDS scale scores between the two groups before and after 4 weeks of treatment was statistically significant(P<0.05).The comparison of SDS scale scores between the two groups after 4 weeks of treatment was statistically significant(P<0.05),and the SDS scale scores of the test group were significantly lower than those of the control group after 4 weeks of treatment.The results indicated that both the test group and the control group had a therapeutic effect on SD patients,and the treatment effect of the test group was better than that of the control group.5.Chinese medicine evidence score scaleThe two groups were not statistically significant before treatment(P>0.05)and were comparable.The intra-group comparison of the TCM Evidence Score Scale scores between the two groups of subjects before and after 4 weeks of treatment was statistically significant(P<0.05).The comparison between the two groups of patients with TCM Evidence Score Scale scores after 4 weeks of treatment was not statistically significant(P=0.34>0.05),indicating that the test and control groups had a therapeutic effect on patients with SD,and the two groups had comparable treatment effects.Conclusion:1.both fine milli-acupuncture and coarse milli-acupuncture have significant therapeutic effects on subthreshold depression of liver-depression and spleen-deficiency type,which can effectively improve the depressive symptoms of subthreshold depressed subjects,alleviate depression and improve the quality of life of patients.2.The therapeutic effect of fine milli-acupuncture was better than that of coarse milliacupuncture in the HAMD-17 and SDS scales,and the therapeutic effect of coarse hoacupuncture and fine milli-acupuncture was comparable in the CES-D and Chinese Medicine Evidence Scale.The treatment of patients with subthreshold depression of liver-depression and spleen-deficiency type was better with fine milli-acupuncture. |
