Objective:1.This study analyzed and summarized the clinical efficacy of "soothing the liver and strengthening the spleen" acupuncture method in the treatment of mild to moderate depression after stroke(liver depression and spleen deficiency type),and evaluated the influence of "soothing the liver and strengthening the spleen" acupuncture method on mood, sleep and quality of life of patients with mild to moderate depression after stroke(liver depression and spleen deficiency type).2.On the basis of the clinical practice experience of the tutor, the clinical treatment ideas and norms of "soothing the liver and strengthening the spleen" acupuncture for the treatment of mild and moderate depression after stroke(liver depression and spleen deficiency type)were summarized and constructed, and the treatment plan of this disease was expanded.Method:A total of 74 patients with liver depression and spleen deficiency PSD meeting the inclusion criteria were collected in this study, and randomly divided into the control group(simple drug treatment group),37 cases, and the treatment group("soothing the liver and strengthening the spleen" acupuncture combined with Western medicine treatment group),37 cases. The control group was given Escitalopram on the basis of basic treatment, and the treatment group was given "soothing the liver and invigorating the spleen" acupuncture treatment on the basis of the control group.Points were selected from men(left),Taichong(affected side),Zusanli(affected side),Pishu(bilateral),Jiaji point,Neiguan(affected side)and Baihui. In the acupuncture treatment,Jiaji point,Pishu point acupuncture,no needle left;Needles were left for 30 minutes after acupuncture at other points, and needles were performed once during the period, each time lasting about 30 seconds, once a day.The drainage method was used for middle phase men(left)and Taichong(affected side),the replenishing method was used for Zusanli(affected side),and the Pingbuping drainage method was used for Neiguan(affected side)and Baihui, and the treatment lasted for 6 days a week, with a total of 4weeks.The changes of TCM syndrome score,Ability of Daily Living Scale(ADL),Hamilton Depression Scale 17-item(HAMD-17)score and National Institutes of Health Stroke Scale(NIHSS)score before and after treatment were recorded in the two groups.According to statistical methods, the score results of the two groups were analyzed, and the therapeutic effect differences between the two groups were compared.Psychological counseling was conducted throughout the treatment. After the treatment, the patient and his family members were instructed to continue physical and psychological rehabilitation at home.Results:1.TCM syndrome score:The TCM syndrome score of both groups decreased before and after treatment(P < 0.01),and after treatment, the combined treatment group had better improvement effect on TCM syndrome symptoms than the control group(P < 0.01).2.BI scale:After treatment, the score of daily living scale in both groups increased compared with that before treatment(P < 0.05),and after treatment, the ADL score in the combined treatment group increased more significantly than that in the control group(P <0.01).3.NIHSS Scale:NIHSS score of both groups decreased after treatment compared with before treatment(P < 0.05),but NIHSS score of combined treatment group decreased more significantly than control group after treatment(P < 0.01).4.Comparison of HAMD scale scores:HAMD scores of the two groups decreased after treatment compared with before treatment(P < 0.01),but the improvement of HAMD scores of the combined treatment group was significantly better than that of the control group after treatment(P < 0.01).Conclusion:1."Soothing the liver and invigorating the spleen" acupuncture combined with Western medicine and Western medicine alone can both improve clinical symptoms, reduce HAMD and NIHSS scores, and improve patients’ daily living ability. The therapeutic effect of the combined treatment group is better than that of the Western medicine alone.2."Soothing the liver and invigorating the spleen" acupuncture combined with Western medicine has a significant effect on improving stroke symptoms in patients with PSD.3."Soothing the liver and strengthening the spleen" acupuncture combined with Western medicine is obviously superior to simple western medicine in improving the TCM syndromes of patients with mild to moderate post-stroke depression(liver depression and spleen deficiency type),so the treatment of PSD according to the syndrome classification has certain clinical reference value.
