Study 1 The past and present of the HB PlasterObjective:To study the theoretical mechanism of traditional Chinese medicine of HB Plaster and the spirit of Zhao Bingnan,a sincere doctor,inherit his innovative spirit and ideas,and provide a better thinking foundation for the clinical treatment of such diseases.Methods:Read through "Jingcheng Great Doctor Zhao Bingnan","Concise Chinese Medicine Dermatology" and combined with clinical contact with the experience of Beijing Hospital of Traditional Chinese Medicine in the treatment of keloids,read the progress research of traditional Chinese and Western medicine in the treatment of keloids,participate in the preparation of HB Plaster and personally experience the external application of HB Plaster,summarize the pharmacological characteristics of HB Plaster in traditional Chinese medicine,and put forward improvement plan speculation,and finally study Zhao Bingnan’s experience,summarize and analyze the background and spiritual source of a generation of famous doctors with such pioneering ideas.Results:The various ingredients in HB Plaster are mainly based on detoxification,clearing heat and channeling.Black vinegar can peel off the epidermis,so that fivefold seed powder,centipede,ice chips,honey can be closely in contact with the skin lesions,the drug is ground into a powder to attack the poison and disperse,relieve pain and circulation,and harvest the sores and muscles with fivefold seed powder;Ice chips are bitter and cold,in this ointment,the main effect is to clear heat and relieve pain;When honey relieves itching,it can also detoxify.Combined with the current treatment progress,the author proposes an improvement plan:one can be combined with a pressure treatment plan,and the other can imitate the principle of cryotechnology innovation,extract traditional Chinese medicine ingredients,and inject to treat keloids.Mr.Zhao Bingnan was young and bumpy,suffering and sick,but "benevolence,skill,diligence,and peace" are his main life creeds.Conclusion:The reason why Zhao Lao can enjoy the reputation of Jinghua and can create the dermatology department of traditional Chinese medicine is closely related to his benevolence,mysticism,diligent study and humility,his youth has suffered from illness,he can better understand the feelings of patients,and has more willingness and ideas to relieve patients’ suffering;His diligent learning attitude,deep understanding of Chinese medicine medicine,diligent in practical operation,and active participation in clinical practice,can obtain its "magic technique";His humble and gentle attitude allows for a harmonious doctor-patient relationship and a deeper discussion of the details of the medication.On top of these four spirits,there is such an innovative method.From the aspects of TCM differentiation and pharmacology,this paper analyzes why HB Plaster can be used to treat keloids.Study 2:Network pharmacology and molecular docking study of HB Plaster for the treatment of keloidsObjective:The network pharmacology and molecular docking research of the drugs in HB Plaster were carried out,and their mechanism of action was analyzed,which provided more data support for clinical use and provided certain directions for subsequent basic experiments.Methods:The network pharmacology of black vinegar,pentapleseed,centipede,honey and ice chips was carried out,and all chemical components in these five drugs were collected by using the TCMSP Chinese medicine system pharmacology database and analysis platform,traditional Chinese medicine syndrome association database(SymMap)and literature search,and the target protein abbreviations were retrieved through the UniProt database.By using the GeneCards database,OMIM database and CTD database,the disease targets of keloids were searched to obtain keloid-related targets and names.The two were then intersected and a protein interaction(PPI)network diagram was drawn through the STRING11.0 database.Then,the core components and core targets in the HB Plaster were screened by designing the recurrence network diagram,and then the core target was intersected with the core protein target of PPI,and the relevant target genes and pathways were understood and analyzed through GO and KEGG enrichment analysis,and finally the molecular docking technology was used to verify the connection activity between the protein receptor and the small molecule ligand.Results:After deduplication,a total of effective active compounds were obtained:1 pentaploid,9 ice chips,5 centipedes,4 black vinegar,47 honey,and 2061 effective drug targets were obtained.GO enrichment analysis obtained 1747 biological processes,90 cellular components and 159 molecular functions.KEGG enrichment analysis yielded 160 signaling pathways,mainly involving fluid shear stress and atherosclerotic pathways,arginine and proline metabolism pathways,HIF-1 signaling pathway(hypoxia-inducible factor 1).In terms of molecular docking,the binding energy of quercetin to MMP2,PGTS2,VEGFA,nicotine to MMP2 and PTGS2,and luteolin to MMP2,PTGS2 and VEGFA was less than-5.0 kcal·mol-1,indicating that these small molecule components bind well to these protein targets.Among them,the binding energy of quercetin to MMP2 and VEGFA,and luteolin to MMP2 and VEGFA was less than-7.0 kcal·mol-1,indicating strong binding activity.Conclusion:The core chemical components in the HB Plaster are Acetic Acid(acetic acid),Quercetin(quercetin),Nicotine(nicotine),Acetaldehyde(acetaldehyde),Luteolin(luteolin),and the core target proteins are MMP2,PTGS2,VEGFA.It may mainly treat keloids by inhibiting unlimited cell proliferation,inhibiting apoptosis,anti-inflammatory,antitumor,antibacterial,anti-infective,may be involved in angiogenesis,apoptosis and other mechanisms. |
