Analysis Of The Efficacy And Prognostic Factors Of Combined Traditional Chinese And Western Medicine In The Treatment Of Sudden Deafness Flat Typ

Purpose:To explore the curative effect of integrated TCM and modern medicine in treating sudden deafness flat type(qi stagnation and blood stasis type),the relationship between the related influencing factors and prognosis,and judge whether the factors can be used as a reliable indicator of prognosis of sudden deafness flat type.Material and method:This study was a retrospective study,and clinical data of 101SSNHL patients(sudden deafness group)were randomly collected from patients diagnosed with flat SSNHL in our hospital from January 2018 to December 2021.Meanwhile,101 healthy people were randomly collected from health examination center of our hospital as the control group.Before treatment,the deafness group and the control group were examined,and the blood routine,liver function,coagulation five items,fasting blood glucose,blood lipid and other experimental indicators were collected.The sudden deafness group received relevant treatment according to the treatment guidelines for sudden deafness(2015),while the control group received no treatment.Pure tone audiothreshold examination was performed on the10 th day for the sudden deafness group,and the efficacy and prognosis of the sudden deafness group were evaluated.In gender,age,with the ears,sick time,period of treatment,whether to have accompany symptoms(tinnitus,aural fullness,vertigo),are associated with disease(diabetes,high blood pressure),TCM syndrome,the related parameters of blood(Fg、albumin、neutrophils、lymphocytes、PLT、FAR、NLR、PLR、FBC、LDL、TG、TC、HDL-C、LDL-C)Conduct statistical analysis,use T test,non-parametric test,Fisher test and Kruskal-Wallis test to calculate Z value and P value,conduct single factor logistics regression analysis,and take single factor(P < 0.05)as the result to further conduct multi-factor logistics regression analysis.To explore the factors that may affect the curative effect and prognosis.Results:1.There were statistically significant differences in the total effective rate among all groups in age,whether accompanied with vertigo,whether accompanied with diseases(diabetes),TCM syndrome and blood parameter NLR value(P < 0.05).There was no statistical difference in the total effective rate among all groups in terms of gender,duration of disease,course of treatment,concomitant symptoms(tinnitus,stuffy ears)and concomitant diseases(hypertension)(P > 0.05).2.Relative blood parameters(Fg,albumin,neutrophils,PLT,lymphocytes,FAR,NLR,PLR,FBC,TG,TC,HDL-C,LDL-C)were compared between flat sudden deafness group and healthy control group after integrated TCM and modern medicine treatment.Fg,albumin,neutrophil,NLR and FBC were significantly different between the two groups(P <0.05).There were statistical differences in NLR value between the control group and the cured group,the effective group,the effective group and the ineffective group(P < 0.05).The value of the sudden deafness group was much higher than that of the healthy control group,and the value of NLR gradually increased with the reduction of the curative effect.There was no significant difference in pairwise comparison among cured group,effective group,effective group and ineffective group(P > 0.05).There were no significant differences in PLR and FAR values of control group,cured group,marked effect group,effective group and ineffective group(P > 0.05).3.Single factor logistics regression analysis was conducted for the sudden deafness group,and the HDL-C value,NLR value,whether accompanied by vertigo,whether accompanied by diabetes were correlated with the prognosis of SSNHL,and there were statistical differences(P < 0.05).In terms of the duration of disease,course of treatment,whether accompanied by symptoms(tinnitus,ear tightness),and whether accompanied by hypertension,There were no statistical differences(P > 0.05).Further multi-factor logistics regression analysis of related factors showed that there were statistically significant differences in vertigo and NLR(P <0.05),while there were no statistically significant differences in HDL-C and diabetes(P >0.05).The OR value of vertigo and NLR value > 1,indicated that vertigo and NLR value might be independent factors affecting the prognosis of SSNHL.Conclusion:1.After integrated TCM and modern medicine treatment of flat type SSNHL,patients with smaller TCM syndrome score had better curative effect;patients with younger age and shorter disease duration had better curative effect;patients with SSNHL with vertigo had worse curative effect.2.The prognosis of patients with flat SSNHL is related to the presence or absence of vertigo,diabetes mellitus,HDL-C and NLR,and the presence or absence of vertigo and NLR can be used as independent factors to evaluate the prognosis of SSNHL.3.NLR value is related to the prognosis of SSNHL.The NLR value of SSNHL patients is higher than that of normal people,and the higher the NLR value,the worse the prognosis.
