| Purpose: Based on the age difference,analyze the onset causes,comorbidities,abnormal physical and chemical indicators,disease location syndrome elements,disease syndrome elements,and the distribution of tophi in male gout patients,so as to provide individualized diagnosis,treatment and prevention of male gouty arthritis Provide theoretical reference.Methods: Collected 145 male patients with gouty arthritis who were hospitalized in the Rheumatology Department of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from January 1,2020 to September 30,2021.For abnormal physical and chemical indicators,tophi formation and clinical four-diagnosis data,refer to the "Syndrome Differentiation Scale" in "Syndrome Differentiation" to determine the patient’s relevant disease location and disease characteristics,enter the data,and analyze the distribution law.Explore the relationship between various factors and age.Results:1.General data distribution: Among the 170 male gout patients,64 were young,52 were middle-aged,and 54 were elderly.2.Distribution of pathogenic factors: The most common pathogenic factors of male gouty arthritis are,in descending order,overeating,alcohol,no obvious incentive,drinking,seafood,fatigue,and cold.There was a statistically significant difference between the young group and the elderly group in the incidence of "excessive food,fat,sweet and thick flavor"(P=0.0076<0.01),and the middle-aged group and the elderly group were in the "excessive food,fatty,sweet and thick flavor".There was a statistically significant difference(P=0.0181<0.05)in the cause of the disease,which showed that the number of people in the young group and the middle-aged group was more than the old group because of overeating.3.The symptoms of gouty arthritis in males are tendons and bones,liver,kidney and spleen in descending order;there is a statistical difference between the young group and the old group in the disease site of "kidney"(P=0.0006 < 0.01),there was a statistically significant difference between the middle-aged group and the elderly group in the “kidney” disease site syndrome(P=0.0065<0.01),which indicated that the “kidney” disease site syndrome factor in the elderly group may be greater than that in the young and middle-aged groups;There was a statistically significant difference between the young group and the elderly group in the disease site "spleen"(P=0.0013 <0.01),and there was a statistically significant difference between the middle-aged group and the elderly group in the "spleen" disease site(P =0.0014<0.01),which means that the syndrome elements of "spleen" disease in the elderly group may be greater than that in the young and middle-aged groups.4.The symptoms of male gouty arthritis are heat,blood stasis,dampness,phlegm,qi deficiency,yin deficiency,yang deficiency,and cold in descending order;There was a statistical difference(P=0.0264<0.05),which showed that the number of occurrences of the disease syndrome element "heat" in the youth group was more than that in the elderly group;There was a statistically significant difference between the two groups(P=0.008<0.01),and there was a statistically significant difference between the young group and the elderly group in the disease syndrome factor of “stasis”(P=0.0012<0.01),indicating that the middle-aged group and the elderly group had a “stasis” factor(P=0.0012 < 0.01).” appeared more frequently than the youth group;there was a statistical difference between the youth group and the elderly group in the disease syndrome of “phlegm”(P=0.0007<0.01),and the middle-aged group and the elderly group in the disease syndrome of “phlegm” There was a statistically significant difference between the two groups(P=0.0242<0.05),indicating that the number of occurrences of "phlegm" in the elderly group was more than that in the middle-aged group and the youth group;there was a statistical difference in the "cold"syndrome element between the youth group and the elderly group.(P=0.0304<0.05),there was a statistical difference between the young group and the old group in the syndrome element of "yang deficiency"(P=0.0170<0.05),indicating that the old group was more likely to have "cold" and "cold" than the young group.Yang deficiency".5.Distribution of comorbidities: The common comorbidities of male gouty arthritis are dyslipidemia,hypertension,NAFLD,renal insufficiency,abnormal liver function,gallbladder stones,kidney stones,coronary heart disease,and pulmonary interstitial changes in descending order.There was a statistical difference in the comorbidity of "hypertension" between the young group and the middle-aged group(P=0.0171 < 0.05),and there was a statistical difference in the incidence of "hypertension" between the young group and the elderly group(P=0.0171<0.05).0 < 0.01),there was a statistical difference between the middle-aged group and the elderly group in the incidence of "hypertension"(P=0.032 < 0.05),which showed the possibility of combined hypertension,the elderly group > the middle-aged group > the young group;There was a statistically significant difference between the young group and the middle-aged group in the comorbidity of "NAFLD"(P=0.0153<0.05),which indicated that the combined NAFLD was possible,and the young group was greater than the middle-aged group;There was a statistically significant difference in the comorbidity of“abnormality”(P=0.0139<0.05),which indicated that it was possible to combine abnormal liver function,and the young group > the old group;the young group and the old group had the complication of “renal insufficiency”.Statistical difference(P=0.0018<0.01),showing the possibility of combined renal insufficiency,the elderly group> the young group;there is a statistical difference between the young group and the elderly group in the comorbidity of "coronary heart disease"(P=0.0002<0.01),there was a statistically significant difference between the middle-aged group and the elderly group in the comorbidity of "coronary heart disease"(P=0.0435<0.05),which indicated that the combined coronary heart disease was possible,and the old group was greater than the young group and the middle-aged group.6.Abnormal physical and chemical indicators: The abnormal physical and chemical indicators of male gout patients appear in descending order: ESR increased,UA increased,CRP increased,TG increased,GGT increased,CREA increased,ALT increased,24 h UUA decreased,24 h UUA increased,CHOL increased,UREA increased.There was a statistical difference in the abnormal physicochemical index of "GGT elevation" between the young group and the middle-aged group(P=0.0179<0.05),and there was a statistical difference in the abnormal physicochemical index of "GGT elevation" between the young group and the elderly group(P=0.0179<0.05).P=0.0161<0.05),which showed that the GGT of male gout patients may be increased,and the young group > the middle-aged group and the elderly group;there was a statistical difference in the abnormal physical and chemical index of "ALT elevation" between the young group and the elderly group(P= 0.0003 < 0.01),which indicated that ALT may be elevated in male gout patients,the youth group > the elderly group;there was a statistical difference between the youth group and the elderly group in the abnormal physicochemical index of “urea elevation”(P=0.0000 < 0.01),the middle There was a statistically significant difference in the abnormal physical and chemical index of "UREA increased" between the aged group and the elderly group(P=0.0076<0.01),which showed that UREA was increased in male gout patients,and the elderly group was greater than the youth group and the middle-aged group.7.There was no statistical significance in the formation of tophi in the young group,middle-aged group and elderly group(P=0.7826﹥0.05).Conclusion:1.The first cause of male gout arthritis is "overeating fat and sweet taste",and it is most common in young and middle-aged patients.2.Male gouty arthritis patients have the most "kidneys" and "spleens" in general,compared with young and middle-aged people,elderly patients have more "kidneys" and "spleens";overall "heat" appears the most in pathogenic evidence;compared with young patients,middle-aged and elderly patients "stasis" appears more;compared with young and middle-aged patients,elderly patients "phlegm" appears more;compared with young patients,elderly patients "cold" and "yang deficiency" appear more.3.The most common comorbidities of gouty arthritis in men are dyslipidemia.The second is hypertension,and the higher the age,the greater the likelihood of concomitant hypertension.It is more common for young patients with gouty arthritis with NAFLD and abnormal liver function;elderly patients with gouty arthritis with renal insufficiency and coronary heart disease are more common.4.In male patients with gouty arthritis,"elevated GGT" and "elevated ALT" are more common in young people;"elevated UREA" is more common in the elderly. |
