| Objective: Collected from liaoning university of traditional Chinese medicine affiliated hospital dermatological department nearly two years treatment of 163 cases of damp and hot accumulate skin type pan eczema cases of hospitalized patients with clinical data,and concludes the onset of eczema patients related situation,the skin lesion characteristics and related factors such as data statistical analysis,and clinical treatment and curative effect evaluation and so on,to explore and summarize the characteristics associated with disease,To provide clinical reference for future eczema diagnosis and treatment.Materials and METHODS: Clinical data of patients with damping-heat generalized eczema who received treatment in the inpatient Department of dermatology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from March 1,2019 to July31,2021 were retrospectively analyzed.Medical records of 163 patients were analyzed according to inclusion and exclusion criteria.General information(such as gender,age,occupation,etc.),disease-related information(such as onset time,course of disease,inducements,skin lesions characteristics,etc.),and laboratory examination results were collected,summarized and analyzed.The factors to be studied were input into Excel spreadsheet,and statistical analysis was conducted using IBM SPSS26.0 statistical software and ancient and modern medical record cloud platform.Results:1.General information: Among 163 patients,99(60.74%)were male and 64 (39.26%)were female;Age distribution ranged from 17 to 88 years old,with an average age of 61.98±15.04 years old.Patients aged 60-79 years accounted for the largest proportion.The morbidity rate was highest among retired personnel.The duration of the disease can be as short as 1 day and as long as 30 years.The mean course of disease was 53.35±77.98 months.The longest length of hospitalization was 23 days,the shortest was 7 days,and the mean length of hospitalization was 13.16±6.60 days.2.Incidence related situation: 28 cases(17.18%)were smokers and drinkers,mostly male;132 patients had underlying diseases(80.98%),including 78 patients with previous cardiovascular diseases;123 cases(75.46%)had a history of drug allergy,most of which were penicillins and sulfonamides.There were 31 patients with seafood,alcohol,fatigue,mosquito bites and other factors before onset.146 patients had red tongue and 64 patients had yellow and greasy tongue.The patients felt severe pruritus,decreased appetite in 10 cases,poor sleep in 144 cases and abnormal stool in 47 cases.3.Skin lesions: there were 163 patients with generalized lesions all over the body,with 130 cases(16.91%)on the trunk and 128 cases(16.64%)on the limbs as the most common lesions,including papules,erythema,scratches and blood scabs.EASI score was 27.15±11.28 before treatment and 6.80±4.93 after treatment.VAS score was 80.83±8.67 points before treatment and 20.40±11.30 points after treatment.4.Laboratory examination: Fasting blood glucose was ≥7.0mmol/L in 47 cases(28.83%).Serum total Ig E increased in 85 cases(52.15%)and eosinophil increased in 87 cases(53.37%).Seventeen cases had abnormal liver enzymes.32 cases had abnormal renal function,including27 cases(16.56%)of uric acid elevation.Abnormal myocardial enzymes were found in 78 cases.Twelve patients had dyslipidemia.113 cases of immune dysfunction.Forty-seven patients were positive for allergens.5.TCM treatment: the first 10 frequency of oral TCM decoction for 163 patients were: Cortex moutan 155 times,rehmanniae rehmanniae 151 times,radix paeoniae saposhniae 143 times,licorice 139 times,Scutellaria baicalensis 133 times,Alisma alisma 126 times,Schizonepeta chinensis 111 times,Cortex albinacea 110 times,tribulus teristus 110 times and gardenia jasminoides 105 times.Drug mostly for cold,cold.The use of bitter,sweet and other medicinal taste.The first three times were liver meridian 1351 times,heart meridian 1055 times and spleen meridian 1035 times.The main effects of TCM are heat and detoxification403 times,heat and dampness 400 times,heat and blood cooling 359 times.6.Cluster analysis of 15 high-frequency drugs: Category 1.Atractylodes macrocephala,Radix Sophorae flavescens,coix seed;Category 2.Bupleurum,Schizonepeta,Gardenia,liquorice,Alisma orientalis;Class 3.Dittany bark,tribulus briar,Scutellaria baicalensis,parsnip,peony bark,rehmannia glutinosa.7.Efficacy: among 163 patients,6 cases(3.68%)were cured,139 cases(85.28%)were significantly effective,and 16 cases(9.82%)were effective.The total effective rate was98.78%.Conclusion:1.In this study,elderly patients were in the majority,and the incidence of elderly patients with metabolic syndrome was high;2.Traditional Chinese medicine decoction with cold,bitter taste,the main heart,liver,spleen meridian.To clear heat,benefit water seepage wet effect drugs mainly.3.Top 10 ingredients of TCM decoction in frequency of use: Cortex moutan,Rehmannia glutinosa,parsnip,licorice,Scutellaria baicalensis,Alisma alismatis,Schizonepeta chinensis,Cortex chinensis,Tribulus teristus and Gardenia jasminoides.4.New formula was obtained after clustering: Class 1 is suitable for patients with dampness-heat skin syndrome of eczema and severe dampness-evil of spleen deficiency.Class 2 is suitable for eczema patients with heat symptoms and fire toxin heat evil.Category 3is suitable for patients with severe itching or blood heat spots attacked by wind evil.5.The clinical effective rate of the patients in this study was 98.78% after traditional Chinese and Western medicine treatment,and the effect was remarkable. |
