Purpose:Through the study of tongue picture of 69 obese patients with type 2 diabetes(diabetes mellitus type 2,T2DM),the objective parameters of tongue picture were compared with fasting blood glucose(FBG),glycated hemoglobin(Hb A1c),triglyceride(triglyceride,TG),total cholesterol(total cholesterol,TC),high density lipoprotein cholesterol(High density lipoprotein cholesterol,HDL-C),low density lipoprotein cholesterol(Low density lipoprotein cholesterol,LDL-C),fasting c-peptide(Fasting C-peptide)to conduct a correlation study to explore the correlation between tongue image and clinically relevant indicators in obese patients with type 2 diabetes mellitus,so as to provide reference for clinical diagnosis.Method Using the Huiyigu Traditional Chinese Medicine Inspection Information Collection and Management System,69 obese T2 DM patients and 67 T2 DM patients with normal weight in the Endocrinology Department of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine were included according to the admission and discharge criteria.Collection and analysis of the tongue image, and the basic information of the patient was collected, the tongue image of the patient was collected by the tongue diagnostic apparatus, and analyzed by the system to obtain the tongue image feature description and tongue image parameters.TG,TC,HDL-C,LDL-C,c-peptide indicators. The spss25.0 statistical software was used to analyze the general data using the chi-square test;the measurement data using the t test, expressed as the mean ± standard deviation, the rate comparison between the categorical variables using the X2 test, the differences between the two groups of tongue images were analyzed and compared,Correlation analysis and multiple linear regression were used to explore the correlation between age, course of disease, laboratory parameters and tongue parameters in overweight/ obese patients with type 2 diabetes, and an association model was established. Results The following results were found by analyzing the correlation between the tongue image and age, gender, body mass index, and laboratory parameters of the included 6 9 obese T2 DM patients. There was no significant difference in gender and age between obese patients with type 2diabetes and normal weight patients with type 2 diabetes(p>0.05),and there was no statistical difference in the frequency of tongue coating and tongue shape between the two groups(p>0.05).The frequency of occurrence of dentate tongue and thick greasy fur in the obesity group was higher than that in the normal weight group, and the frequency of cracked tongue and thin fur in the normal weight group was higher than that in the obese group. There were statistical differences between the groups in Lab-a tongue quality(p < 0.01),but no statistical differences in other tongue parameters.A total of 69 obese patients with T2 DM were included, and there was no statistical difference in gender, age and disease course(p>0.05).Among them,42 were male and 27 were female. The frequency of abnormal tongue shape and tongue coating in male patients was higher than that in female patients. The age distribution of the patients ranged from 24 to 78 years old, with an average age of 51.19± 12.83 years old.Among them, there were 22 young patients(≤44 years old),accounting for 31.9% of the total number, and 27 middle-aged patients(45-59),accounting for the total number of patients.39.1%,and 20 elderly patients(≥60 years old),accounting for 29% of the total. The frequency of cracked tongue and thin fur in middle-aged patients was higher than that in young and elderly patients, and the frequency of dentate tongue and greasy fur in elderly patients was higher than that in green and middle patients.33 patients with disease duration ≤5 years,20 patients with disease duration 6 to 10 years,16 patients with disease duration > 10 years, and obese T2 DM patients with disease duration ≤ 5 years have a higher frequency of cracked tongue, dentate tongue, thick coating and greasy coating than Patients with a disease course of 6 to 10 years and a disease course of more than 10 years.Tongue distribution in obese T2 DM patients:(1)Tongue shape distribution: 23 cases of cracked tongue(accounting for 33.3%)had the highest frequency, followed by 22 cases of dentate tongue(accounting for 31.9%),and 20 cases of moderate tongue(accounting for2 9.0%)4 cases of tongue pricking(5.8%)(2)Distribution of tongue coating: On the tongue coating, the thick and greasy coating occurs more frequently(50.7%).In conclusion, the tongue characteristics of obese patients with T2 DM are cracked tongue, and thick and greasy coating is more common, and more with dentate tongue. The correlation analysis between laboratory indicators in obese T2 DM patients was as follows:fasting blood glucose(FBG)was negatively correlated with full tongue Lab-b,positively correlated with tongue coating Lab-b,and correlated with full tongue Lab-L,Lab-a,tongue coating Lab-L,Lab-a no correlation.Fasting c-peptide was negatively correlated with tongue quality Lab-a,but not with full tongue quality Lab-L,Lab-b,tongue coating Lab-L,Lab-a,Lab-b.Glycated hemoglobin(Hb A1c)was not correlated with any tongue parameters.Correlation analysis was performed on triglyceride(TG),total cholesterol(TC),high density lipoprotein(HDL-C),low density lipoprotein(LDL-L)and parameters of tongue quality and tongue coating. The whole tongue quality Lab-b was negatively correlated, but had no significant correlation with tongue quality Lab-L,tongue quality Lab-a,tongue coating Lab-L,tongue coating Lab-a,and tongue coating Lab-b.TC was positively correlated with the tongue coating Lab-L,but had no significant correlation with the tongue coating Lab-L,the tongue coating Lab-a,the tongue coating Lab-b,the tongue coating Lab-a,and the tongue coating Lab-b.LDL-C was positively correlated with tongue quality Lab-a and tongue coating Lab-a,but had no significant correlation with tongue quality Lab-L,tongue quality Lab-b,tongue coating Lab-L,and tongue coating Lab-b.There was no significant correlation between HDL-C and various tongue parameters. The following 3 equations were obtained by regression analysis:FBG= 18.844-0.929×tongue Lab-b,TC=2.32+0.115×tongue Lab-L,TC= 1.798+0.118×tongue Lab-a.Conclusion1.Through the analysis of the tongue image of obese patients with T2 DM,this study found that the tongue shape of obese patients with type 2 diabetes is more common with cracked tongue and dentate tongue, and the abnormal tongue coating is more common with thick and greasy coating, suggesting that the pathogenesis of phlegm-dampness in obese patients with T2 DM is prominent.By comparing the tongue image of obese T2 DM patients and normal weight T2 DM group,it was found that the frequency of greasy fur and tooth marks in the obese T2 DM patients was higher than that of the normal weight group, suggesting the influence of phlegm-dampness pathogenic qi on the tongue picture of the patients.2.Through the correlation study between the tongue picture and age, course of disease, and laboratory indicators of obese T2 DM patients,it was found that there is a correlation between full tongue Lab-b and FBG and TG,and full tongue Lab-a and fasting c-peptide.,LDL-C is correlated, tongue coating Lab-L is correlated with TC,tongue coating Lab-b is correlated with FBG,and Lab-a is correlated with LDL-C...
