| Purpose:This study adopted the method of Bayesian mesh meta-analysis to systematically study the effects of Proprietary Chinese medicines on various aspects of chronic glomerulonephritis,in order to obtain the best intervention measures and provide higher quality evidence-based evidence for the clinical treatment of chronic glomerulonephritis by TCM.Materials and Methods:Randomized controlled trials(RCTs)of different Chinese patent medicines in the treatment of chronic glomerulonephritis were collected by computer searching Chinese Journal Full-text Database(CNKI),Wanfang Medical Database,Chinese Scientific and Technological Journal Database(VIP),China Biomedical Literature Database(Sino Med)and other databases.The retrieval period is from January 2010 to December 2021,and the collection of books is searched by hand in order to obtain relevant references.Literature screening,data acquisition and cross-checking were carried out independently by two researchers,and the quality of included literature was evaluated according to the Cochrane Systematic Review manual.ADDIS software was used for mesh meta-analysis of data.Results:Eventually into 39 randomized controlled trial,a total of 3560 patients with chronic glomerulonephritis,involved with 39 references,a total of 5 involves proprietary Chinese medicine,including: Huangkui capsule,Bailing capsule,Jinshuibao capsule,tripterygium wilfordii glycosides tablets,Kunxian capsule and four outcome indicators,including: the total effective rate,serum creatinine,blood urea nitrogen,24 hours urinary protein quantitative.The results of mesh meta-analysis showed that:1.The total effective rate was a binary variable,and with OR as the effect size,it could be seen from the figure that,compared with conventional treatment of Western medicine,the plan of Chinese patent medicine combined with conventional treatment of Western medicine was superior to conventional treatment of western medicine alone(P <0.05).2.Serum creatinine was MD as the effect size.As can be seen from the results in the figure,the measures of Chinese patent medicine combined with conventional western medicine were better than those of conventional western medicine alone(P <0.05).There were no statistically significant differences among the other interventions in the figure.3.MD was used as the effect size of urea nitrogen.Compared with conventional treatment,bailing capsule + conventional treatment(MD:-1.73,95%CI:-2.78,-0.67)and Huangkui capsule + conventional treatment(MD:-1.88,95%CI:-2.75,-1.05),the difference was statistically significant(P <0.05).There was no significant difference among other interventions.4.24 H urine protein was a continuous variable,and MD was selected as the effect size.It can be seen from the figure that compared with conventional treatment in Western medicine,the combination of Chinese patent medicine and conventional treatment in Western medicine was superior to conventional treatment in western medicine alone(P <0.05).ADDIS probability ranking results are as follows:1.The probability of total efficiency is ranked from high to low as follows: Jinshuibao capsule +conventional treatment of Western medicine(Rank6 probability 0.75)> Kunxian capsule+conventional treatment of Western medicine(Rank6 probability 0.10)> Tripterygium wilfordii polyglycosides tablet +conventional treatment of Western medicine(Rank6probability 0.08)> Huangkui capsule + conventional treatment of Western medicine(Rank6probability 0.04)= Baling capsule + conventional treatment of Western medicine(Rank6probability 0.04)> conventional treatment of Western medicine(Rank6 probability 0.00).3.The probability of urea nitrogen is ranked from high to low: Huangkui capsule + conventional treatment of Western medicine(Rank6 probability 0.31)> Kunxian capsule +conventional treatment of Western medicine(Rank6 probability 0.25)> Bailing capsule +conventional treatment of Western medicine(Rank6 probability 0.22)> Jinshuibao capsule +conventional treatment of Western medicine(Rank6 probability 0.12)= Tripterygium wilfordii polyglycosides tablet + conventional treatment of Western medicine(Rank6probability 0.12)> conventional treatment of Western medicine(Rank6 probability 0.00).4.The probability of 24 H urinary protein was ranked from high to low: Jinshuibao capsule +conventional treatment of Western medicine(Rank6 probability 0.48)> Kunxian capsule +conventional treatment of Western medicine(Rank6 probability 0.44)> Balling capsule +conventional treatment of Western medicine(Rank6 probability 0.06)> Tripterygium wilfordii polyglycosides tablet + conventional treatment of Western medicine(Rank6probability 0.02)> Huangkui capsule + conventional treatment of Western medicine(Rank6probability 0.02)> conventional western medicine treatment(Rank6 probability 0.00).Conclusion:1.Chinese patent medicine can effectively improve total effective rate、serum creatinine、urea nitrogen and 24-hour urinary protein in patients with chronic glomerulonephritis.2.Total effective rate: all kinds of Chinese patent medicine combined with conventional treatment have certain effects,among which jinshuibao capsule combined with conventional treatment of Western medicine has the best effect.3.Serum creatinine: various Chinese patent medicines combined with conventional treatment have certain effects,among which Jinshuibao capsule combined with conventional treatment of Western medicine has the best effect.4.Urea nitrogen: various Chinese patent medicines combined with conventional treatment have certain effects,among which Huangkui capsule combined with conventional treatment of Western medicine has the best effect.5.24-hour urine protein: all kinds of Chinese patent medicine combined with conventional treatment have certain effects,among which Jinshuibao capsule combined with conventional treatment of Western medicine has the best effect.6.Different Proprietary Chinese medicines have different therapeutic effects for different intervention measures,but Jinshuibao capsule has the most prominent effect.Clinicians should choose the most suitable proprietary Chinese medicine treatment plan according to the specific situation. |
