| Purpose:Based on the latent structure model(Lantern 5.0),the characteristics and distribution of syndrome elements of type 2 diabetic nephropathy(G3a stage)were discussed,which provided an objective and reliable basis for clinical diagnosis and treatment of type 2 diabetic nephropathy(G3a stage).Materials and methods: From March to September 2021,patients with type 2 diabetic nephropathy(G3a stage)who visited the Endocrinology ward and outpatient department of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine were selected,and 60 patients who met the inclusion and exclusion criteria of this study were selected as this study.sub-study object.The general condition,BMI value,urinary albumin/urine creatinine(UACR)and other clinical indicators and clinical symptoms of the patients were collected,and the information of the four medical diagnoses of the patients was completely recorded.Using the latent structure analysis software Lantern 5.0,the latent structure model was established for the symptom data of type 2 diabetic nephropathy(G3a stage),and the latent structure analysis was carried out to interpret the syndrome element characteristics of the latent variables.According to the principle of cumulative mutual information,a subset of significant variables were selected,the syndrome elements of the disease were extracted,and comprehensive cluster analysis was performed to summarize the distribution characteristics of TCM syndromes of type 2 diabetic nephropathy(G3a stage).Results:1.60 cases of type 2 diabetic nephropathy were included,including 28 females,accounting for46.6%,and 32 males,accounting for 53.3%.The average duration of diabetes in the included patients was 12.44(±11.236)years,and the average age was 62.23(±9.781)years,mainly middle-aged and elderly patients.The physical manifestations were dominated by normal body shape and overweight body shape,accounting for 45% and 41.66%,respectively,11.67% of obese patients,1.67% of thin patients,and the average BMI was 24.62(±3.311)kg/m2.The UACR stage of the included patients was mainly A2,including 41 cases,accounting for68.3%;A3 stage,including 12 cases,accounting for 20%;A2 stage,including 7 cases,accounting for 11.6%.Among the different stages of UACR,the A1 stage was 6.15(±1.87)mg/g,the A2 stage was 44.68(±6.51)mg/g,and the A3 stage was 540.93(±298.28)mg/g.2.The disease sites suggested in the 60 cases of diabetic nephropathy were spleen,kidney,heart,liver and stomach;the pathological factors were qi deficiency,blood stasis,yin deficiency,dampness,heat,yang deficiency,qi stagnation and blood deficiency.3.Interpret the syndromes of the Z1-Z3 latent structure model obtained after comprehensive clustering,and the results suggest that the disease location is in the spleen,kidney,heart,and liver;the disease is a mixture of deficiency and excess,heat,cold and heat;pathological factors include Qi deficiency,blood stasis,phlegm-dampness,yin deficiency,fire,yang deficiency;syndromes include spleen deficiency and dampness syndrome,liver stagnation transforming fire syndrome,yin and yang deficiency syndrome,and blood stasis blocking collaterals syndrome.Conclusion:1.Type 2 diabetic nephropathy(G3a stage)is mainly located in the spleen and kidney,and is related to the heart and liver;2.Based on the evaluation results of the latent structure model,qi deficiency,phlegm-dampness,yin deficiency,yang deficiency,blood stasis,and heat are the main syndrome elements of type 2 diabetic nephropathy(G3a stage).Syndrome types include spleen deficiency and dampness,liver stagnation transforming into fire,yin and yang deficiency,and blood stasis blocking collaterals.3.Spleen deficiency is the central link in the occurrence of type 2 diabetic nephropathy(G3a stage). |
