Clinical Observation On The Treatment Of Phlegm-heat Obstructing Lung Syndrome In Ventilator-associated Pneumonia With Ma Xing Shi Gan Tang Combined With Xuan Bai Cheng Qi Tan

Objective: To observe the clinical efficacy of Maxingshigan Decoction combined with Xuanbaichengqi Decoction in the treatment of ventilator-associated pneumonia with phlegm-heat blocking the lung,and evaluate its effectiveness and safety.Methods: A total of 40 cases who met the diagnostic criteria of VAP and belonged to the syndrome of phlegm-heat obstructing the lung according to TCM syndrome differentiation were selected and randomized into a treatment group and a control group using the randomized controlled study method,with 20 cases in each group.The control group was treated with conventional western medicine,and the treatment group was additionally treated with Maxingshigan Decoction combined with Xuanbaichengqi Decoction based on the control group.The inflammation indexes,clinical pulmonary infection score,APACHEⅡ II score,TCM syndrome score and curative effect,mechanical ventilation time and adverse reactions were observed before and after the treatment.Results: 1.The age,gender,BMI and basic diseases of the two groups of patients before treatment are comparable(P>0.05).2.After treatment,the scores of TCM syndromes between groups were statistically significant(P<0.05);There was no significant difference in the curative effect of TCM syndromes(P>0.05).The white blood cell count,PCT and CRP were statistically significant(P<0.01).After treatment,there was statistical significance in CPIS between groups(P<0.05).There was significant difference in APACHEⅡ score between groups(P<0.05).The mechanical ventilation time between groups was statistically significant(P<0.05).Conclusion: Maxingshigan Decoction combined with Xuanbaichengqi Decoction can improve clinical symptoms,reduce inflammatory index,reduce CPIS and APACHEⅡscores,shorten mechanical ventilation time,and have certain safety and effectiveness.
