Psoriasis vulgaris,known as "Baibi disease" in traditional Chinese medicine,is a common inflammatory skin disease with erythematous scales.The disease has a long course,is easy to recur,and is lingering and difficult to heal,which has the characteristics of dry skin,hypertrophy,scaling and itching,affecting about 2%to 3%of the world’s population.It not only affects the beauty,but also causes serious psychological and economic burden to patients.Western medicine has been proved to have definite curative effect in clinic,but the side effects on patients are inevitable and expensive.Psoriasis is called "psoriasis dry","snake lice","baibi","snake wind" and "psoriasis pine" in ancient Chinese books.Chinese medicine has a long history and good efficacy in the treatment of psoriasis.Chinese medicine has accumulated rich experience in the treatment of psoriasis,which not only has a lasting effect.but also has less side effects.Based on the clinical practice,according to the classics of traditional Chinese medicine,clinical practice experience,modern related literature and the characteristics of psoriasis vulgaris with blood-heat syndrome and skin rash,this study objectively evaluated the safety and effectiveness of "analogous medicine selection" external treatment of Liangxue Jiedu Runji Ointment in the treatment of psoriasis with blood-heat syndrome.OBJECTIVE:To objectively evaluate the clinical efficacy,skin barrier function improvement,TCM symptoms,quality of life,self-rating health scale improvement and safety of Liangxue Jiedu Runji Ointment in the treatment of Baibi with blood-heat syndrome,and to provide scientific basis for the application of TCM external treatment.METHODS:The symmetrical skin lesions of psoriasis vulgaris patients with blood-heat syndrome were selected as the target lesions.The symmetrical skin lesions were divided into treatment group and control group by random control method,and the random number table was used to divide symmetrical lesions into treatment group and control group,giving treatment of Liangxue Jiedu Runji ointment and placebo ointment respectively.The efficacy evaluation and target skin lesions photography were performed every two weeks for four times with treatment course lasting eight weeks.The efficacy determination and safety testing were conducted after eight weeks.The primary evaluation indexes were skin lesion area and severity index,and the secondary evaluation indexes includes skin barrier function test,visual analogue score(VAS),target lesion area,dermatology quality of life index(DLQI),health status questionnaire(SF-36),the main clinical symptoms of TCM indicators.Results:A total of 50 patients were recruited,of which 46 patients completed the clinical observation,4 patients fell off for unknown reasons.In the treatment group,1 case(2.17%)was cured,36 cases(78.26%)were markedly effective,9 cases(19.57%)were effective and 0 cases(0%)were ineffective.In the control group,1 case(2.17%)was cured,17 cases(36.96%)were markedly effective,15 cases(32.61%)were effective and 13 cases(28.26%)were ineffective.Target skin lesion score has significant statistical difference(P<0.05),the treatment group is better than the control group;drug safety monitoring,treatment group has 2 cases,control group has 4 cases of adverse events related to treatment,considering being related to acute stage of psoriasis with blood-heat syndrome,there is no obvious abnormality in laboratory examination.There were significant statistical differences in the improvement of skin barrier function(P<0.05).After 8 weeks of treatment,there were significant differences in TCM symptoms,DLQI,VAS,BAS and SF-36(P<0.05).CONCLUSION:Liangxue Jiedu Runji Ointment is effective and safe in treating psoriasis vulgaris with blood-heat syndrome.It can effectively improve the skin barrier function and the quality of life of patients.
