Objective To analyze the characteristics of body fat distribution in patients with OSAHS and to explore the correlation between body fat distribution and the severity of OSAHS.Methods A total of 237 patients with snoring symptoms during sleep were enrolled from July 2021 to August 2022.According to the results of sleep monitoring,they were divided into simple snoring group of 69 cases,mild to moderate OSAHS group of 77 cases,and severe OSAHS group of 91 cases.Gender,age,height,weight,body mass index(BMI),neck circumference(NC),waist circumference(WC),hip circumference(HC),waist-to-hip ratio(WHR),modified Mallampati score,Epworth sleepiness scale(ESS)were collected.Inbody570 bioelectrical impedance analyzer were performed to measure body composition.To analyze the characteristics of body fat distribution in OSAHS patients and to explore the correlation between body fat distribution and the severity of OSAHS.Results 1.There were no significant differences in gender and age among three groups(P>0.05);ESS and modified Mallampati score in severe group were higher than mild to moderate group and simple snoring group(P<0.05);The BMI,NC,WC,HC,WHR,percentage of body fat,percentage of trunk fat and percentage of limb fat in severe and mild to moderate groups were higher than simple snoring group,and the above indexes in severe group were higher than mild to moderate group(P<0.05);The percentages of muscle,trunk muscle,limb muscle,total water content and protein in severe and mild to moderate groups were lower than simple snoring group,and the above indexes in severe group were lower than mild to moderate group(P<0.05);The percentage of inorganic salt and bone mineral content in the severe group was lower than mild to moderate group and simple snoring group(P<0.05).2.BMI,NC,WC,HC,WHR,percentage of body fat,percentage of trunk fat and percentage of limb fat were all positively correlated with AHI(r=0.528,0.514,0.573,0.241,0.524,0.623,0.651,0.572,P<0.05).3.Multivariate Logistic regression analysis showed that the influencing factors for OSHAS included BMI(OR=1.222,95%CI=1.021~1.464,P=0.029),NC(OR=1.230,95%CI=1.033~1.464,P=0.020),WC(OR=1.080,95%CI=1.013~1.151,P=0.018)and percentage of trunk fat(OR=1.635,95%CI=1.337~2.000,P=0.000).4.AUC was used to prove the diagnostic efficiency of the 4 indexes for screening OSAHS and they were listed according to the order of AUC as: percentage of trunk fat(AUC=0.892),BMI(AUC=0.872),WC(AUC=0.856),NC(AUC=0.777).The combined diagnosis of multiple indexes(AUC=0.937)has higher diagnostic value than that of single indexes.Conclusion 1.Patients with OSAHS have increased fat content and mainly abdominal fat accumulation.2.The percentage of trunk fat is most closely related to the severity of OSAHS,and combined with anthropometric indicators,it has higher diagnostic value.
