Objective1.The purpose of this study was to investigate the status quo of maternal breastfeeding self-efficacy and maternal and infant attachment within 6 months after delivery,and to explore the longitudinal changes in time and their influencing factors.2.To explore the relationship between postpartum maternal breastfeeding self-efficacy and maternal and infant attachment,in order to provide a theoretical basis for clinical development of accurate nursing intervention strategies,improve breastfeeding self-efficacy and maternal and infant attachment level,promote breastfeeding rate and prolong breastfeeding time and improve infant health.Methods In this prospective cohort study,questionnaires were used to collect 433 parturitions at 42 days postpartum,and clinical data of 403 parturitions at 3 months postpartum and 368 parturitions at 6months postpartum were collected through follow-up.In December 2020 to June 2021,USES the convenience sampling method to select clinic and Nantong in Nantong University affiliated hospital of postpartum maternal and child health care of postpartum maternal of outpatient medical as the research object,and the scene to fill in the questionnaire,the Breastfeeding Self-efficacy Short Form Scale breastfeeding self-efficacy,Maternal Postnatal Attachment Scale measurement from mother to child,The general data questionnaire,Pittsburgh Sleep Quality Index,Postpartum fatigue Scale and Edinburgh Postpartum Depression Scale were used to collect the data of sociodemographic,sleep quality,postpartum fatigue and postpartum depression.The data of social demography,breastfeeding self-efficacy and maternal and infant attachment were followed up at 3 and 6 months postpartum by wechat,questionnaire or telephone,respectively.SPSS software was used for statistical analysis.Repeated measures ANOVA was used to analyze the longitudinal changes and trends of breast-feeding self-efficacy and mother-infant attachment over time.Multiple linear regression was used to analyze the relationship between mother-infant attachment and breastfeeding self-efficacy.Pearson correlation analysis was used to analyze the correlation between breastfeeding self-efficacy and maternal and infant attachment.Results1.The scores and total scores of various dimensions of breast-feeding self-efficacy and mother-infant attachment increased successively at 42 days,3 months and 6 months postpartum.The total score of breast-feeding self-efficacy at 42 days postpartum was(50.65±13.35)points,3 months postpartum was(52.04±13.02)points,and 6 months postpartum was(54.03±14.07)points.The total score of maternal and infant attachment was(74.78±7.39)at 42 days postpartum,(74.81±7.44)at 3 months postpartum,and(76.38±7.25)at 6 months postpartum.The results of repeated measures ANOVA showed that time had statistical significance on the total scores of breast-feeding self-efficacy and mother-infant attachment(P<0.05).2.Stepwise analysis using multiple linear regression model showed that adequate self-perceived breastfeeding,maternal-infant attachment,daily breastfeeding times,postpartum depression,difficulties encountered during feeding and planned breastfeeding time were independent influencing factors of 42 days postpartum breastfeeding self-efficacy,while maternal and infant attachment,self-perceived adequate breastfeeding times per day,sleep quality and bottle use during breastfeeding were independent influencing factors of breastfeeding self-efficacy at 3 months postpartum.Maternal and infant attachment,self-perceived adequate breastfeeding and daily breastfeeding times are independent factors affecting the self-efficacy of breastfeeding at 6months after delivery.3.After Pearson correlation analysis,the correlation coefficient r between breastfeeding self-efficacy and maternal and infant attachment at42 days,3 months and 6 months postpartum was respectively 0.447,0.486,0.475(P < 0.001).Conclusions1.The overall levels of breastfeeding self-efficacy and maternal and infant attachment scores were relatively high,and gradually increased at42 days,3 months and 6 months after delivery,and the total scores showed a linear relationship with time,respectively,indicating that breastfeeding self-efficacy and maternal and infant attachment levels gradually increased over time.2.Maternal and infant attachment is one of the influencing factors of breast-feeding self-efficacy at 42 days,3 months and 6 months after delivery.It is clinically possible to improve the level of maternal and infant attachment to improve breast-feeding self-efficacy,so as to promote breast-feeding.3.There was a significant positive correlation between breastfeeding self-efficacy and maternal and infant attachment,and the higher the maternal and infant attachment level was,the higher the breastfeeding self-efficacy level was.
