Effect Of Surgical Timing On The Prognosis Of Elderly Patients Treated With Minimally Invasive Surgery For Intertrochanteric Fractures

Objective(s): After the occurrence of elderly intertrochanteric fractures,the same surgical and internal fixation method(PFNA)was used to perform surgical treatment at different surgical times,and the relevant influencing factors of different patients after surgery were analyzed to clarify whether the operation could be completed as soon as possible,so as to reduce the preoperative waiting time of patients,reduce clinical complications,and enable patients to return to normal life functions early.Methods: Elderly patients with intertrochanteric fractures of the femur who underwent surgical treatment(PFNA)in the Department of Emergency Trauma Surgery of XX Hospital from January 2020 to June 2022 were collected as the research subjects,and with reference to the diagnostic criteria for senile intertrochanteric fractures in the 8th edition of the textbook "Surgery",the patient’s history of morbidity,clinical examination and auxiliary examinations(hip X-ray,CT)were used as the basis for diagnosis,and a total of 76 patients were finally included for clinical research after inclusion criteria and exclusion criteria.According to the different timing of surgery during surgical treatment after injury,the patients included were divided into two groups: experimental group and control group,the experimental group was patients who received surgical treatment within 96 hours of onset,and the control group was patients who received surgical treatment after 96 hours of onset.With the help of IBM SPSS Statistics 26 software system,through retrospective research,the relevant clinical data of the two groups of patients in the treatment process,such as the length of surgery,intraoperative blood loss,total hospital stay,total hospital cost,the score of the patient’s postoperative hip function recovery(Harris score)and clinical complications after treatment,to compare the clinical differences caused by different surgical timing of the two groups of patients to clarify the safer and more effective operation time.Results: Through the analysis of the relevant data of the two groups,it was found that there were no statistical differences in age,sex,lateral disposition,fracture type,osteoporosis,post-injury admission time,and anesthesia ASA score between the two groups(P>0.05),indicating that the two groups were comparable.The operation time and intraoperative blood loss of the two groups were compared,and no statistical difference was found(P>0.05),indicating that patients undergo surgical treatment at different surgical times,which has no significant effect on the operation time required for surgical operation and the amount of intraoperative bleeding.The functional scores,postoperative complications and mortality of the postoperative hip joints of the two groups were compared and analyzed,and no statistical differences(P>0.05)were found,indicating that the choice of operation timing had no significant effect on the recovery of postoperative hip function,postoperative complications and mortality.The hospitalization time and hospitalization cost of the two groups were compared and analyzed,and it was found that there was a statistical difference between the data of the two groups(P<0.05),indicating that the hospitalization time and hospitalization cost of the experimental group were significantly lower than those in the control group.Conclusion(s): Through the analysis of relevant clinical data and postoperative follow-up,it is known that the choice of the timing of surgery will not affect the surgical process,nor will it affect the postoperative hip recovery,postoperative complications and mortality.However,if the patient’s physical condition allows,early surgical treatment can shorten the patient’s hospital stay,enable the patient to perform joint function exercise early,and return to normal life early.It can reduce the cost of hospitalization,relieve the financial pressure of the patient’s family,and also improve the utilization rate of beds.
