| Objective: Observe the clinical effect of accelerated rehabilitation surgery in the management of elderly patients with intertrochanteric fractures.Methods: This study retrospectively analyzed 60 patients with intertrochanteric fractures of the femur who were admitted to the Department of Orthopedics of the North Theater General Hospital and Northeast International Hospital from January 2018 to December 2019.The surgical methods were all PFNA.The 30 patients admitted from January 2018 to December 2018 were used as the traditional group,and the traditional intervention program was adopted.From January 2019 to December 2019,30 patients admitted to the ERAS group were given interventions in accordance with the concept of accelerated rehabilitation surgery.The clinical data of the two groups of patients were analyzed,and the average hospital stay,postoperative VAS pain score,postoperative hip Harris score,postoperative ADL score,postoperative complications and postoperative hemoglobin values were compared to determine Clinical efficacy.Results:(1)Compared with the traditional group(16.43 ± 2.11 d),the hospitalization time(11.23 ± 3.88 d)of the ERAS group was statistically significant(P <0.05).(2)The healing time of surgical incision in ERAS group(10.93 ± 1.93)was shorter than that in traditional group(14.5 ± 1.28),the difference was statistically significant(P <0.05).(3)The VAS scores of the ERAS group on the 1st and 5th day after surgery were(3.1 ± 1.56,2),which was lower than that of the traditional group(4.9 ± 1.61,3),the difference was statistically significant(P <0.05).(4)Harris score of ERAS group in the first and third month after operation was(81.10 ± 5.26,91.23 ± 4.21),which was higher than that of traditional group(72.20 ± 5.47,80.60 ± 5.68),the difference was statistically significant Significance(P <0.05).(5)The ADL scores of the ERAS group in the first and third months after surgery were(54 ± 10.37,87.83 ± 7.73),which was higher than that of the traditional group(44.47 ± 12.50,74.67 ± 9.37),the difference was statistically significant Significance(P <0.05).(6)The incidence of complications in the ERAS group was 13.3%,lower than 36.7% in the traditional group,the difference was statistically significant(P<0.05).(7)The number of hemoglobin lost in the ERAS group on the first and fourth days after surgery was(14.47 ± 11.14,10.67 ± 13.23),which was less than that of the traditional group(25.97 ± 15.06,25.40 ± 15.34),the difference was statistically significant Significance(P <0.05).Conclusion: The application of accelerated rehabilitation surgery in elderly patients with intertrochanteric fractures during the perioperative period has a better clinical effect than traditional treatment methods.After the pain,accelerate the healing of the surgical incision,allow patients to perform functional exercise early,and accelerate the recovery of hip joint function,which is of great significance to accelerate the overall recovery of elderly patients with intertrochanteric fractures. |
