Clinical Observation On Tangwangtongluo Decoction Combined With Leizumab In The Treatment Of Diabetes Retinopathy

ObjectiveTo observe the clinical efficacy of Tangwangtongluo Decoction combined with Leizhumab in the treatment of Diabetes Retinopathy(DR)with phlegm and blood stasis blocking collaterals,spleen and kidney deficiency in terms of the best corrected vision,fundus signs,foveal thickness of macular,TCM syndrome scores,and provide a better clinical plan for the treatment of DR.MethodsThis study is divided into theoretical research and clinical research.The theoretical research includes "explaining the current situation and prospect of DR from modern medicine" and "visualizing the current situation of traditional Chinese medicine in treating DR from bibliometrics".The clinical study collected 46 DR patients(62 eyes in total)who met the diagnostic criteria in the ophthalmology clinic and inpatient department of the Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine from December 2020 to December 2022,including 31 eyes in the treatment group and 31 eyes in the control group.Both groups of patients were treated with intravitreal injection of rapuzumab,and the treatment group was treated with Tangwang Tongluo Decoction;Intravitreal injection once a month for 3 consecutive months,traditional Chinese medicine decoction for 4 weeks as a course of treatment,and oral administration for 3 consecutive courses.After the course of treatment,the curative effects and effects of the two treatment schemes on patients’ visual function protection,fundus signs,and TCM syndromes were compared.Establish a database and conduct statistical analysis.At the same time,the intelligent system is used to quantitatively analyze the improvement of the two fundus signs of bleeding and exudation in DR patients,and optimize the efficacy evaluation of traditional Chinese medicine in a data-based wayResultsThrough reviewing the research on DR in modern medicine,we can see that intravitreal injection of rapzumab is an effective treatment for DR,but it has the defects of high price and need to be injected repeatedly.Through the visual analysis of the current situation of traditional Chinese medicine treatment of DR from the perspective of bibliometrics,it can be seen that oral Chinese medicine treatment of DR has its unique advantages,"visceral deficiency and collateral obstruction" is the basic pathogenesis of Qi,and the treatment of DR through the combination of Chinese and Western medicine has its practical significance.1.Comparison of baseline data:There was no statistical difference between the two groups in terms of age,duration of diabetes,and HbAlc(P>0.05).2.Best corrected visual acuity(BCVA):After 3 months of treatment,the vision of patients in both groups was improved,and there was no significant difference between the two groups(P>0.05).3.Symptoms of Fundus:After 3 months of treatment,there was a significant difference in fundus hemorrhage between the two groups(P<0.05),no significant difference in fundus exudation(P>0.05).Comprehensive observation of the three major fundus signs of microangioma,neovascularization and non-perfusion zone,the effective rate was 80.64%in the treatment group and 45.16%in the control group,with a significant difference(P<0.05).4.Centre Macular Thickness(CMT):After 3 months of treatment,there was no significant difference between the two groups(P>0.05),but the significant efficiency of the treatment group was 80.65%,and that of the control group was 48.39%,with a significant difference(P<0.05).5.Syndrome Score of traditional Chinese medical science:after 3 months of treatment,there was significant difference between the two groups(P<0.05).The apparent efficiency of the treatment group was 35.48%,and that of the control group was 3.22%,with significant difference(P<0.05).It suggests that the treatment group has more significant improvement in TCM symptoms than the control group.ConclusionTangwang Tongluo Decoction combined with Leizumab can improve many clinical symptoms of DR patients with phlegm-stasis blocking collaterals and spleen and kidney deficiency,safe and effective,it can improve and promote the absorption of fundus hemorrhage and the fundus signs such as microangioma,neovascularization and nonperfusion zone.The efficacy of reducing macular edema is more stable,and has its unique advantages in improving systemic symptoms,which is superior to the use of rapuzumab alone.
