Clinical Observation Of Yangxue Xifeng Decoction In The Treatment Of Uremic Pruritus Of Blood Deficiency And Wind Dryness Type

Objective: By observing the effects of Yangxue Xifeng Decoction combined with loratadine tablets and calamine lotion on high-sensitivity C-reactive protein,interleukin-6,visual analogue score,5-D pruritus scale score,TCM syndrome score and safety index in patients with uremic pruritus of blood deficiency and wind dryness type,the clinical therapeutic effect of Yangxue Xifeng Decoction combined with loratadine tablets and calamine lotion on patients with uremic pruritus of blood deficiency and wind dryness type and its influence on related inflammatory factors were clarified,and its possible mechanism of action was discussed.Provide a basis for clinical application.Methods: According to the inclusion criteria,64 patients with uremic pruritus of blood deficiency and wind dryness type who underwent maintenance hemodialysis in Shijiazhuang Hospital of Traditional Chinese Medicine from December 2021 to December 2022 were selected.The subjects were divided into control group and experimental group by random number table method,32 cases in each group.The control group was treated with calamine lotion combined with oral loratadine tablets,and the experimental group was treated with Yangxue Xifeng decoction on the basis of the control group.4 weeks for a course of treatment,continuous observation of two courses.The changes of high-sensitivity C-reactive protein,interleukin-6 and visual analogue score,5-D pruritus scale score,TCM symptom score and safety index of the control group and the experimental group before and after 8 weeks of treatment were observed to evaluate the clinical efficacy,and the shedding cases and adverse events during the study were recorded in detail.According to the principle of statistics,the collected data were input into SPSS25.0 and statistically analyzed to evaluate its efficacy and preliminarily analyze the relevant influencing factors.Results: 1.Before treatment,the general data(gender,age,dialysis age,KT/V,hs-CRP,IL-6),primary disease,VAS score,5-D score and TCM syndrome score of the two groups were not statistically significant(P >0.05).2.VAS score was positively correlated with hs-CRP(r =0.907,P <0.01)and IL-6(r = 0.928,P < 0.01).The correlation was statistically significant.The 5-D score was positively correlated with the levels of hs-CRP(r = 0.830,P < 0.01)and IL-6(r = 0.851,P < 0.01).The correlation was statistically significant.3.After treatment,the VAS score and 5-D total score of the experimental group and the control group were lower than those before treatment,and the difference was significant(P < 0.01).The improvement of the experimental group was better than that of the control group,and the difference was statistically significant(P < 0.01).4.After treatment,the levels of hs-CRP and IL-6 in the experimental group and the control group were significantly lower than those before treatment(P < 0.01),and the improvement of the experimental group was significantly better than that of the control group(P < 0.01).5.After treatment,the total scores of TCM syndromes in the experimental group and the control group were decreased,the difference was significant(P <0.01),and the improvement of the experimental group was better than that of the control group,the difference was statistically significant(P < 0.01).6.After treatment,in terms of the effective rate of pruritus,the total effective rate of the experimental group was 90.00%,and the total effective rate of the control group was 66.67%.The curative effect of the experimental group was better than that of the control group,and the difference between the two groups was statistically significant(P < 0.05).7.After treatment,in terms of the effective rate of TCM curative effect,the total effective rate of the experimental group was 93.33%,and the total effective rate of the control group was 63.33%.The curative effect of the experimental group was significantly better than that of the control group,and the difference between the two groups was statistically significant(P <0.01).8.Observation of safety data: During the clinical observation,there were no abnormalities in liver function,electrolyte,blood routine,urine routine,stool routine,electrocardiogram and other safety observation indicators in each group,and no adverse reaction events were reported.Conclusions: 1.Yangxue Xifeng Decoction combined with western medicine can reduce the TCM syndrome score of UP patients with blood deficiency and wind dryness and improve clinical symptoms.2.Yangxue Xifeng Decoction combined with western medicine can reduce the VAS score and 5-D pruritus scale score of patients,reduce the symptoms of skin pruritus and improve the quality of life of patients.3.Yangxue Xifeng Decoction combined with western medicine can reduce the levels of hs-CRP and IL-6 and improve the microinflammatory state of patients.4.The micro-inflammatory state is common in patients with uremic pruritus,and the levels of hs-CRP and IL-6 are closely related to the micro-inflammatory response of patients,which can be used as indicators to evaluate the efficacy of uremic pruritus.5.No obvious side effects occurred during the treatment of the research,which is safe and reliable,and has good clinical application value.
