According to the World Health Organization incomplete statistics the main cause of global human death is cardiovascular disease,the more common type of clinical is myocardial ischemia caused by myocardial ischemia reperfusion injury.In recent years,with people’s sub-healthy lifestyle habits,it gradually presents the characteristics of youthfulness and high incidence.Clinical treatment is performed by coronary artery bypass grafting and percutaneous coronary intervention with cyclooxygenase inhibitors,calcium channel blockers,beta-receptor antagonists and other drugs.To a certain extent,the exacerbation of the disease is delayed,but there are also side effects such as anti-platelet drug resistance,frequent bleeding events and postoperative chest tightness and chest pain.Therefore,the development of drugs with low side effects and high efficacy for the treatment of myocardial ischemia-reperfusion injury is still one of the key directions of medical research.During the development of myocardial ischemia,blood rheology changes,which in turn stimulates platelet aggregation and activation,while platelets recruit neutrophils,leukocytes,and macrophages in the blood resulting in further aggravation of the inflammatory response,thus further aggravating myocardial injury.Anti-platelet drugs such as aspirin and Tegretol are commonly used clinically to alleviate thrombosis,but they also increase the risk of internal bleeding.Therefore,the search for safer and more effective drugs remains an important goal to address clinical needs.Buyang Huanwu Decoction is a classic formula for the treatment of ischemic heart disease.It has been shown that it effectively alleviates ischemic myocardial injury by reducing calcium overload,inhibiting cardiomyocyte apoptosis,and reducing vascular endothelial injury in the treatment of coronary heart disease,but there are fewer studies on its antimyocardial ischemic effect by improving platelet function.In this paper,we further investigated the protective effect of Buyang Huanwu Decoction against myocardial ischemia and reperfusion injury and explained its mechanism of action.Objective: To study the protective effect of Buyang Huanwu Decoction treatment based on improving platelet function against myocardial ischemia-reperfusion injury and to investigate its possible mechanism of action.Method(s): 1.To establish a model of myocardial ischemiareperfusion injury in rats,and to study the protective effect of Buyang Huanwu Decoction against myocardial ischemia-reperfusion injury in rats by using echocardiographic evaluation,pathological observation and determination of serum cytokine levels.2.To study the anti-platelet activation effect of Buyang Huanwu Decoction by detecting platelet parameters,adhesion rate,agglutination rate and platelet factor content in rats.3.To detect the expression of platelet receptors and related signaling pathway proteins,and to explore the mechanism of action of Buyang Huanwu Decoction against platelet activation.The expression of platelet receptors and related signaling pathway proteins were examined to explore the mechanism of the anti-platelet activation effect of Buyang Huanwu Decoction.Result(s): 1.Buyang Huanwu Decoction(low,medium and high doses)significantly improved the degree of heart infarction and cardiac function,reduced serum levels of cardiac enzymes and inflammatory factors,and improved cardiac pathology,thereby improving myocardial ischemia-reperfusion injury.2.Buyang Huanwu Decoction(high doses)significantly improved blood rheology,reduced platelet adhesion/agglutination rate,and reduced the levels of related inflammatory factors to exert anti-platelet activation effects.3.The results of the study showed that Buyang Huanwu Decoction could down-regulate the expression of CD36,SRC,ERK5 and P38 proteins,indicating that Buyang Huanwu Decoction could exert anti-platelet activation effect through regulating CD36-SRC-ERK5 signaling pathway to exert anti-myocardial ischemia-reperfusion injury effect.Conclusion: The mechanism of the anti-inflammatory and antiplatelet activation effects of Buyang Huanwu Decoction may be through the regulation of the CD36-SRC-ERK5 signaling pathway to exert myocardial protective effects. |
