Objective:West China Psychiatry Association somatic symptoms classification scale is designed to solve the difficulties in the diagnosis and treatment of psychosomatic diseases in general departments.It is designed to conduct a comprehensive psychological assessment of various somatic symptoms and help non-psychological doctors to quickly identify and classify psychosomatic diseases.In this study,WCPA somatic symptoms classification scale,dyspepsia symptom score and Pittsburgh Sleep Quality Index were used to evaluate the physical symptoms and sleep situation of functional dyspepsia patients,so as to understand the psychopsychological components behind the physical symptoms and sleep status of functional dyspepsia patients in the department of gastroscopy outpatient of general hospital,and improve the clinical management level.Methods:A total of 113 FD patients who met the Rome IV diagnostic criteria were selected from the outpatient department of Hebei general Hospital from May 2022 to December 2022.According to their dyspepsia symptoms,they were divided into three groups: postmeal distress syndrome group(PDS group),epigastrie pain syndrome group(EPS group)and overlapping symptoms group(PDS+EPS group),and the dyspepsia symptoms were scored.The WCPA somatic symptoms classification scale was used to classify the somatic symptoms of the three groups of patients,and analyze the different characteristics of the physical symptoms of each group.The Pittsburgh Sleep Scale was used to assess the patients’ sleep status and understand the status of sleep disorders.Statistical methods such as descriptive analysis,nonparametric test,Chi-square test or Fisher’s exact probability method were used for data analysis and processing.Result:1.A total of 113 FD patients were included in this study,with an average age of 52.00(43.00,58.00)years.The male to female ratio was42:71.There were 24 cases(21.2%)in primary school or below,48 cases(42.5%)in junior middle school,18 cases(15.9%)in senior high school,and 23 cases(20.4%)with bachelor’s degree or above.113 FD patients were divided into PDS group(33 cases),EPS group(20 cases),and PDS+EPS group(60 cases).There were no significant differences in age,gender,education level and occupation composition among the three groups of FD patients(P > 0.05).2.The 113 FD patients were evaluated by the somatic symptom classification scale.The results showed that:(1)104 cases(92,1%)of the FD patients had somatic symptoms.The main somatic symptoms of the 104 patients were: 66 cases(63.5%)of emotional somatic symptoms,including41 suppressive somatic symptoms,accounting for 62.1% of emotional somatic symptoms.(2)The main somatic symptoms and the secondary somatic symptom combination type include the suppressive somatic symptoms and imaginative somatic symptoms of 14 cases(13.5%);imagintive somatic symptoms and suppressive somatic symptoms of 14cases(13.5%);suppressive somatic symptoms and cognitive somatic symptoms of 14 cases(13.5%);agitated somatic symptoms and suppressive somatic symptoms of 13 cases(12.5%);suppressive somatic symptoms and agitated somatic symptoms of 11 cases(10.6%).3.Results of WCPA Physical Symptom Classification Scale in patients with different subtypes of FD: the positive rate of suppressive somatic symptoms was significantly different among the three groups(P=0.001),and the frequency of inhibitive physical symptoms in EPS group was significantly lower than that in PDS group(P=0.045)and PDS+EPS group(P=0.001).The positive rate of agitated somatic symptoms was significantly different between groups(P=0.000),and the frequency of agitated somatic symptoms in PDS group was significantly lower than that in EPS group(P=0.012).There was a statistically significant difference in the positive rate of imaginative somatic symptoms between the groups(P=0.002),and the frequency of imaginary somatic symptoms in the PDS+EPS group was significantly higher than that in the PDS group(P=0.001)and EPS group(P=0.008).There was a statistically significant difference in the cognitive rate of imaginative somatic symptoms between the groups(P=0.020),and the frequency of cognitive somatic symptoms in the PDS+EPS group was higher than that in the PDS group(P=0.009).4.The total score,suppressive,agitation,imagination and cognitive somatic symptom factors of WCPA somatic symptom classification scale were statistically significant among the three groups(P<0.05),and the score of PDS+EPS group was significantly higher than that of PDS and EPS groups(P<0.05).5.The dyspepsia symptom score of 113 cases of FD patients was 3.00(2.00,3.00),which was positively correlated with the total score of WCPA Somatic Symptom Classification Scale(P<0.05).The dyspepsia symptom scores of FD patients in PDS group,EPS group and PDS+EPD group were2.00(2.00,3.00),2.00(2.00,3.00)and 3.00(2.00,3.00),respectively,which were positively correlated with the total score of WCPA somatic symptom scale(P<0.05).6.There were 13 cases(39.4%),8 cases(40.0%)and 45 cases(75.0%)of the PDS group,EPS group and PDS+EPS group with sleep disorders,respectively,and the difference between groups was statistically significant(χ2=14.500,P=0.001).The frequency of sleep disorders in PDS+EPS group was significantly higher than that in PDS group and EPS group.Conclusion:1.FD patients are mostly middle-aged women with low educational level.2.FD patients generally have psychological abnormalities manifested by physical symptoms,and the combination of psychological disorders are various.A variety of psychological problems coexist,the severity is different,increase the complexity of the disease,suggesting that multidimensional intervention should be taken into account in the treatment.3.Depressive somatic symptoms were common in PDS patients and agitation somatic symptoms were common in EPS patients.In the treatment should have some emphasis,give individualized treatment.4.FD overlapping group had severe physical symptoms,poor quality of life,and high prevalence of sleep disorders.5.The higher the dyspepsia score of FD patients,the more serious the psychological problems of FD patients. |
