Clinical Observatiaon On Baihe Dihuang Decoction Modified In Treating Type 2 Diabetes Mellitus With Deficiency Of Both Qi And Yin

Purpose: By observing the influence of Baihe Dihuang decoction modified on FPG,2h PBG,Hb A1 c,TG,TC,LDL-C,HDL-C,HOMA-IR indexes of type 2 diabetes with deficiency of both qi and yin,as well as on TCM symptom scores,to evaluate its clinical effectiveness.Methods: A total of 70 patients who met the inclusion criteria and ward of endocrinology department of the city Hospital of Traditional Chinese Medicine from January 2021 to December 2021 were selected as the participants in this study,while divide them into therapy group and control group.All the two groups were given diet and exercise therapy for diabetes.The therapy group was given the basis of diabetes diet therapy with formula of Baihe Dihuang decoction modified,taken with 200 ml,twice a day,once before breakfast and dinner.In the control group,on the basis of diet and exercise therapy for diabetes,metformin tablets were added,0.5g each time,three times a day,and taken orally before meals.Three course of therapy was 12 weeks.The two groups were treated for one course respectively.Including FPG,2h PBG,Hb A1 c,TG,TC,LDL-C,HDL-C,FINS,HOMA-IR etc.comparing before and after therapy of the effective therapy rates,and the efficacy was evaluated according to TCM syndrome integral scale.Using SPSS 26.0 statistical software to analyze and process all data,to contrast the difference of the clinical efficacy between two groups.Result :1.Comparison of total effective rate of clinical efficacy between the two groups: Comparing total clinical response rate,the clinical response rate was 88.2% in the therapy group and 63.6 % in the control group,the therapy group is more better than control group,indicating a statistically meaningful difference(P < 0.05).2.Comparing the total points of TCM syndrome in the therapy group decreased notably meaningful between after therapy and before therapy(P<0.01);After therapy,the total integral between the two groups was statistically significant(P<0.01).in the control group,decreased compared to that before treatment,with a statistical difference(P<0.01).3.Comparison of single TCM syndrome scores: After treatment,compared with before treatment,the individual integral TCM syndromes in the treatment group,the symptoms were improved,with statistical significance(P<0.05);In the control group,the TCM syndrome scores of 3items(dry throat and dry mouth,thirst and drinking,frequent urination)were better than before,with statistical significance(P<0.05);Compared with the scores of traditional Chinese medicine syndromes of 6 items(fatigue,overeating and hunger,shortness of breath and lazy speech,spontaneous sweating and night sweating,five upset heat,tongue and pulse),there was no significant decrease compared to the previous period,with no statistical significance(P>0.05).Compared with the control group,after treatment,there was statistically significant difference in the scores of7 TCM syndromes in the treatment group(fatigue,dry mouth and throat,overeating and hunger;shortness of breath and lazy speech;spontaneous sweating and night sweating,five heart upset heat,tongue and pulse)(P<0.05);the 2 items(dry mouth and throat,frequent urination)had no statistical significance(P>0.05).4.Comparison of blood glucose indicators between the two groups:There were difference in FPG,2h PBG and Hb A1 c between the two groups before and after therapy(P<0.05).After therapy,there was no differences,FPG,2h P and Hb A1 c decreased at the same extent between the two groups,with no statistical(P>0.05).5.Comparison of HOMA-IR between the two groups: Indexes of HOMA-IR decreased significantly lower than before(P<0.05);After treatment,the decrease of HOMA-IR in the two groups was similar,with no statistical significance(P>0.05).6.Comparison of blood lipid indicators between the two groups:between the two groups,TG,TC and LDL-C reduced meaningfully before and after therapy(P<0.05),HDL-C had no meaningful change(P>0.05);After therapy,the quota of TG and LDL-C were meaningful(P<0.05),but TC,HDL-C was no meaningful(P>0.05).7.Safety comparison: During the clinical observation,there were no abnormalities in the safety indicators of the patients and no adverse situation in both groups.Conclusion: 1.The Baihe Dihuang decoction modified can reduce the levels of FPG,2h PBG,Hb A1 c,TG,TC,LDL-C and HOMA-IR in type 2diabetes patients with deficiency of both qi and yin.There was no adverse reactions occurred during administration.2.The Baihe Dihuang decoction modified can improve the TCM clinical symptoms of type 2 diabetes patients with deficiency of both qi and yin,and regulate the level of blood lipids,which is better than oral metformin tablets used alone.
