| Objective: By collecting information on the first page of the cases of hospitalized type 2 diabetes mellitus(T2DM)and its co-morbid elderly patients in a tertiary hospital in Guangxi,the relationship between case mix,grouping and hospitalization costs of different T2 DM hospitalized elderly co-morbid patients was analyzed,which is conducive to scientific and reasonable case grouping and case mix,providing a scientific basis for the payment of medical costs of co-morbid patients and reducing the economic burden of disease of hospitalized elderly co-morbid patients.Methods: In this study,we retrospectively collected case home information of elderly and different comorbidities of T2 DM patients aged ≥65 years who were hospitalized in a tertiary care hospital in Guangxi from 2013-2021.The number of hospitalization days and hospitalization costs were analyzed for different case groups.The main categories included descriptive statistics,correlation analysis,t-test,ANOVA,Mann-Whitney U test,and multiple linear regression.Statistical analysis of the data was performed using SPSS 26.0 with a test level of α=0.05.Results:(1)The proportion of patients with co-morbidities was high,89%,while only 11% of patients without co-morbidities,50% of patients with 1co-morbidity,34% of patients with 2 co-morbidities,and only 5% of patients with ≥3 co-morbidities;as the number of co-morbidities increased,so did the number of hospital days and total hospital costs of elderly patients hospitalized with T2DM(P<0.05);(2)comorbidities differences in the number of hospital days and total hospital costs for different total number of patients(P<0.05);(3)Among the number of 1 comorbidity,the total hospitalization costs of T2DM+cerebral infarction,T2DM+coronary atherosclerotic heart disease,and T2DM+chronic kidney disease were significantly higher than those of T2DM+hypertension(P<0.05);(4)Among the 2 numbers of comorbidities,the total hospitalization costs were significantly higher for T2DM+hypertension+cerebral infarction and T2DM+hypertension+coronary atherosclerotic heart disease than for T2DM+hypertension+chronic kidney disease(P<0.05);(5)There was no statistical difference between the total cost of hospitalization for ≥3 number of comorbidities(P>0.05);(6)Multiple linear regression results showed that total hospitalization costs were higher for male patients than for female patients,lower with increasing age,higher for patients with urban residents’ basic medical insurance,higher for patients with longer hospitalization days,and higher for patients with a higher number of comorbidities(P<0.05).Conclusion:The composition ratio of elderly patients with T2 DM comorbidities was much higher than that of patients with T2 DM mono-morbidities,and hypertensive patients accounted for a great proportion of patients with T2 DM comorbidities.The total hospitalization costs of elderly patients with T2 DM comorbidities showed significant differences in the number of different diseases,and the total hospitalization costs of different case combinations of the same disease also differed. |
