Objective: According to analyze the influencing factors of the prevalence and blood glucose control rate of type 2 diabetes among the elderly aged 65 years and above in Nigang community,Shenzhen,so as to provide a clinical reference for the prevention,diagnosis and treatment of diabetes in the community.Method: This study was a cross-sectional study based on physical examination data.1.Subjects: According to the inclusion and exclusion criteria,elderly people aged 65 years and above who participated in the physical examination in Nigang Community from January 1,2021 to December 31,2021 were recruited as subjects.2.Sectionalization:The population was divided into four groups by age,65-69,70-74,75-79,and ≥ 80 years old.3.Research method : Diabetes history,dietary habits,exercise conditions,height,weight,waist circumference,blood lipid,fasting blood glucose,glycosylated hemoglobin1 were collected by health questionnaire collection,physical examination and laboratory examination.The Chi-square test and multivariate analysis were used for statistical analysis.Results: 1.A total of 1551 residents with an average age of 69.79±4.65 years old were included in this study,including 721 males with an average age of 70.14±5.60 years old.There were 830 females with an average age of 69.48±4.67 years.Among the 1551 subjects,283 were diagnosed with diabetes,with a detection rate of 18.25%;Among 283 patients with diabetes,220 patients had unsatisfactory blood glucose control and 63 patients had satisfactory control,with a blood glucose control rate of 22.3%.2.Analysis of factors influencing the prevalence of Type 2 diabetes in the elderly :Univariate analysis showed that there were no significant differences in gender,BMI,dyslipidemia,frequency of drinking,frequency of physical exercise and dietary habits between diabetic and non-diabetic groups(P> 0.05).There were significant differences in age,waist circumference,hypertension and smoking status(P< 0.05);logistic regression analysis showed that age,waist circumference,hypertension and smoking were risk factors for diabetes(P<0.05).Among them,Subjects aged 70-74 years and 80 years and older had significantly higher risk of diabetes than subjects aged 65-69 years(OR: 1.438,95%CI:1.050-1.971,p=0.024),(OR: 1.913,95%CI: 1.103~3.316,p=0.021),waist circumference(OR: 1.022,95%CI:1.007~1.038,p=0.004),hypertension(OR: 3.819,95%CI:2.897-5.038,p<0.001),had quit smoking(OR: 2.331,95%CI:1.212~4.482,p=0.011).3.Analysis of blood glucose control rate in the elderly with type 2 diabetes mellitus:Univariate analysis showed that there were no statistically significant differences in gender,age,waist circumference,BMI,smoking status,frequency of drinking,frequency of physical exercise and dyslipidemia between the satisfied and dissatisfied groups(P> 0.05).The difference between hypertension and dietary habits was statistically significant(P<0.05);Multivariate logistic regression analysis showed that hypertension(OR: 0.470,95%CI: 0.243-0.908,p=0.025)was a risk factor for satisfactory blood glucose control.Conclusions:Age,waist circumference,high blood pressure and smoking are risk factors for type 2 diabetes in community residents,and high blood pressure is a risk factor affecting blood sugar control. |
