| Objective: Based on data mining technology,this topic collates and summarizes the clinical literature of acupuncture and moxibustion treatment diseases from 2006 to 2020,obtains the disease spectrum of acupuncture and moxibustion from 2006 to 2020,and extracts the dominant diseases.At the same time,by comparing the disease spectrum of acupuncture and moxibustion from 1978 to 2005,the development and changes of the disease spectrum of acupuncture and moxibustion were sorted out,the causes of the change of the disease spectrum were analyzed,and the future application direction and research hotspot of acupuncture and moxibustion therapy were prospected.Method: Literature on acupuncture and moxibustion in China Biomedical Literature Database,China Journal Full-text Database,WANFANG Database and VIP Database(from January 1,2006 to December 31,2020)were searched.Import all the collected documents into the document management software Note express 3.2 to establish a preliminary database.According to the inclusion and exclusion criteria and the classification and statistical methods of disease spectrum,the disease spectrum of acupuncture and moxibustion and "clinical literature database of diseases treated by acupuncture and moxibustion" were obtained from2006 to 2020.Then,the frequency was taken as the basis for the evaluation of appropriate diseases,and the appropriate disease evaluation criteria were drawn up according to the actual data of disease spectrum,and the appropriate diseases were summarized by grouping for statistics and comparison.The diseases were divided into dominant diseases,common diseases and general diseases.At the same time,the disease spectrum of acupuncture and moxibustion from 1978 to 2005 was compared with that of acupuncture and moxibustion from 2006 to 2020,and the changes of the disease spectrum of each system,the specific disease and the frequency of the same disease were analyzed in detail,and the reasons for the changes were discussed.Results:1.acupuncture and moxibustion and moxibustion disease spectrum from 2006 to 2020A total of 78474 compliant literature,including 564 diseases,were included.2.Classification results of acupuncture and moxibustion and moxibustion disease spectrum by frequency from 2006 to 2020Based on the frequency,the dominant disease system is musculoskeletal system,connective tissue disease,and nervous system disease;The dominant diseases are cerebrovascular diseases and sequelae,cervical spondylosis,lumbar disc herniation,facial nerve paralysis,knee osteoarthritis,sleep disorders,hemiplegia,shoulder periarthritis,depression,diabetes and complications,dysphagia,migraine,dysmenorrhea,infantile cerebral palsy,vertigo,obesity,constipation,urinary retention,hiccup.3.Application of acupuncture and moxibustion therapy from 2006 to 2020From the perspective of disease systems,there is a significant difference in the number of literature among different disease systems,with 5 disease systems accounting for more than 5% and 9 disease systems accounting for less than 2%;From the perspective of specific symptoms,the disease spectrum obtained in this study consists of 564 diseases,of which 29 diseases have a frequency greater than 500,accounting for 5.1% of the disease spectrum,but their frequency exceeds 3/5 of the total;There are 453 diseases with a frequency of less than100,accounting for 80% of the disease spectrum,but their frequency only accounts for 1/10 of the total.4.Analysis of changes in the application of acupuncture and moxibustion therapy in recent 15 yearsFrom the perspective of disease system,tumor diseases,ear and mastoid diseases,muscle system diseases and mental diseases are growing faster than other diseases.nervous system disease,endocrine diseases,digestive system diseases,urogenital system diseases and circulatory system diseases are growing at the same rate as the whole.Respiratory system and injury diseases fluctuate greatly but are growing as a whole.Blood and hematopoietic organ diseases,pregnancy,childbirth and puerperal diseases Skin and subcutaneous tissue diseases,eye and appendage diseases,infectious diseases,and parasitic diseases show varying degrees of atrophy;In terms of specific diseases,the frequency of knee osteoarthritis,sleep disorders,shoulder hand syndrome,infantile cerebral palsy,constipation,hemiplegia,dysphagia,tinnitus and other diseases is much higher than that of other diseases,and the frequency of obesity,urinary retention,hiccup,herpes zoster,dementia,acne,trigeminal neuralgia and other diseases is on the decline.5.Analysis of changes in disease spectrum of acupuncture and moxibustionOn the whole,23018 articles were included in the acupuncture and moxibustion spectrum from 1978 to 2005,and 78612 articles were included in the acupuncture and moxibustion spectrum from 2006 to 2020.It can be seen that the clinical literature of acupuncture and moxibustion has increased significantly in the past 15 years;From the perspective of system disease spectrum,the top five diseases in the disease spectrum from1978 to 2005 are: nervous system,musculoskeletal system and connective tissue diseases,urogenital system,digestive system,and respiratory system.From 2006 to 2020,the top five disease spectrums were the muscular system,nervous system,mental system,urogenital system and digestive system,suggesting that the nervous system,muscular system and digestive system have always been the dominant system and research focus of acupuncture and moxibustion therapy,and the research on mental system diseases has increased significantly in recent 15 years;From the number of diseases,the muscle system has the highest number of newly added diseases,reaching 20,followed by the nervous system with16.The system with the most deleted diseases is infectious diseases,with 12 types deleted,followed by eye diseases,with 10 types deleted;From the perspective of specific symptoms,there were a total of 461 symptoms in the disease spectrum from 1978 to 2005,and 564 symptoms in the disease spectrum from 2006 to 2020.Compared to the disease spectrum from1978 to 2005,the disease spectrum from 2006 to 2020 reduced 62 symptoms and added 173 new symptoms.In addition,research on 23 existing diseases has significantly declined,while research on 61 diseases has significantly increased.Conclusion:(1)Acupuncture and moxibustion therapy has a wide range of clinical indications,and the indications are expanding.However,the application intensity of different disease systems and different diseases varies greatly.Although 80% of acupuncture and moxibustion indications have been applied in clinical practice,the frequency is low.The diseases with more clinical applications only account for 5% of the disease types.(2)Based on frequency distribution and changes in literature over the years,the dominant disease systems are musculoskeletal and connective tissue diseases,as well as neurological disorders.In addition,mental and behavioral disorders have developed rapidly in recent years,becoming a new research hotspot and potentially becoming a new dominant disease system.Based on the disease frequency,the disease categories of the acupuncture and moxibustion spectrum were classified.The dominant diseases were cerebrovascular diseases and sequelae,cervical spondylosis,lumbar intervertebral disc herniation,facial nerve paralysis,knee osteoarthritis,sleep disorders,hemiplegia,shoulder periarthritis,depression,diabetes and complications,dysphagia,migraine,dysmenorrhea,infantile cerebral palsy,vertigo,obesity,constipation,urinary retention,hiccup.(3)This study found that cerebrovascular diseases and sequelae,cervical spondylosis,lumbar intervertebral disc herniation,facial nerve paralysis,periarthritis of shoulder,urinary retention,hiccup,infantile cerebral palsy,and migraine are always the dominant diseases and research focuses of acupuncture and moxibustion therapy.In recent 15 years,the frequency of literature on acupuncture and moxibustion treatment of knee osteoarthritis,sleep disorders,hemiplegia,dysmenorrhea,depression,diabetes and its complications,dysphagia,vertigo,obesity,constipation has significantly increased,becoming a new research hotspot in recent years.(4)The main reasons for the change of acupuncture and moxibustion disease spectrum are: the development of acupuncture and moxibustion theory and the innovation of acupuncture and moxibustion instruments,the application and development of acupuncture and moxibustion translational medicine,the extensive implementation of integrated traditional Chinese and western medicine,the support of policies and the continuous improvement of industry standards,the change of disease incidence rate and the change of people’s needs. |
