Retrospective Analysis Of Laparoscope-assisted Radical Gastrectomy Of Lymph Nodes In The 8p Group

Objective:To analyze the influencing factors of lymph nodes metastasis in group 8p and explore the safety of laparoscopic assisted distal gastrectomy combined with lymph nodes dissection in group 8p.Method:From March 2017 to March 2019,104 patients underwent laparoscopic assisted radical resection of distal gastric cancer in the Department of Gastrointestinal,Pancreatic,Hernias,and Abdominal Wall Surgery of Shanxi Provincial People’s Hospital.Among them,52 patients underwent standard D2 radical resection,and 52 patients underwent standard D2 radical resection plus lymph node dissection in group8 p.By comparing the clinicopathological data of 52 patients undergoing lymph node dissection in group 8p,the influencing factors of lymph node metastasis in group 8p were analyzed,To explore the safety of laparoscopic assisted distal gastrectomy combined with lymph node dissection in group 8p by comparing intraoperative and postoperative data between the two groups.Result:Of the 52 patients who underwent lymph nodes dissection in group 8p,10 had metastasis,with a metastasis rate of 19.2%(10/52).Univariate analysis showed that there was no significant correlation between age,gender,T stage,and lymph nodes metastasis in group 8p,while patients with tumors≥5cm,poorly differentiated tumors,and larger N stage had a higher probability of lymph nodes metastasis in group 8p (P<0.05);Multivariate logistic regression analysis showed that tumor≥5cm(P=0.014,95% CI: 1.624-72.846)and tumor low differentiation(P=0.011,95% CI: 0.018-0.593)were independent risk factors for lymph nodes metastasis in group 8p.Laparoscopic assisted radical resection of distal gastric cancer with lymph nodes dissection in group8 p took a relatively long time(P<0.05),but there was no significant difference in intraoperative bleeding,first postoperative exhaust time,first postoperative fluid intake time,postoperative hospital stay,number of intraoperative lymph nodes dissection and(P>0.05).Conclusion:Among the factors related to lymph nodes metastasis in group 8p,tumor≥5cm and tumor low differentiation were independent risk factors;Laparoscopic assisted radical resection of distal gastric cancer with group 8p lymph nodes dissection is safe and feasible.
