Objective:By studying the serum growth stimulation expressed gene 2(ST2)protein levels of different periodontal conditions and serum uric acid(UA)levels of different periodontal conditions in patients with coronary heart disease(CHD),to explored the correlation of periodontal conditions with serum ST2 and with serum UA levels in coronary heart disease patients,providing clinical evidence for the correlation among periodontitis and CHD,To provide coronary heart disease with a new basis for the diagnosis and treatment.Methods:From March 2021 to December 2022,in the department of Cardiology,the The Fifth Clinical Hospital of Shanxi Medical University,among the hospitalized patients who were first diagnosed with CHD through coronary angiography,in strict accordance with the inclusion and exclusion criteria and with respect for the individual wishes of the patients,a total of 102 patients who volunteered for periodontal examination were randomly selected as the study subjects.After signing the informed consent form,a questionnaire survey and periodontal examination were conducted,including probing depth(PD),Clinical attachment loss(CAL),bleeding index(BI),calculus index(CI),debris index(DI),and plaque index(PLI).According to the "2018 New International Classification of Periodontal and Implant Periodontal Diseases" standard,and based on the results of patients’ periodontal examinations,the study subjects were fell into three groups: periodontal health group(18 cases),gingivitis group(18 cases),and periodontitis group(68 cases);The patients with coronary heart disease associated with periodontitis were fell into three groups according to the "1999 Periodontitis Classification Method" and the degree of periodontitis inflammation: mild periodontitis group(25 cases),moderate periodontitis group(23 cases),and severe periodontitis group(20 cases).Record the questionnaire survey results,basic clinical data,periodontal examination results,and detect the serum ST2 and serum UA levels of subjects in each group.Compare the relationship between different periodontal condition with ST2 protein in serum and with serum UA in patients with CHD,and conduct statistical analysis.Results:1.Analysis of periodontal status of research objects with CHD in each groupAmong 102 research objects,16 were in the periodontal health group(15.69%);18patients with gingivitis(17.65%);68 patients(66.67%)in the group with periodontitis,25 patients(24.51%)in the group with mild periodontitis;23 patients with moderate periodontitis(22.55%);There were 20 patients(19.61%)in the group with severe periodontitis.2.Comparison of general data of research objects between groupsThere weren’t prominent difference in age,gender,BMI,blood pressure(systolic and diastolic blood pressure),and fasting blood glucose among subjects with coronary heart disease among the three groups: periodontal health,gingivitis,and periodontitis(P>0.05),between the five groups of periodontal health,gingivitis,mild periodontitis,moderate periodontitis,and severe periodontitis,there were also no significant difference(P>0.05).3.Comparison of serum ST2 and serum UA levels among different periodontal status groups in research objectsThere were a statistically prominent difference in serum ST2 and serum UA levels in the middle of the three groups of research objects : the periodontal health group,the gingivitis group,and the periodontitis group(P<0.05).There were a statistically prominent difference between the two groups compared with the periodontal health group and the gingivitis group(P<0.05);The difference between the group with periodontitis and the group with gingivitis were statistically prominent(P<0.05);There weren’t prominent difference between the gingivitis group and the periodontal health group(P>0.05).Comparison of serum ST2 and serum UA levels in research objects with different periodontal conditions showed that there were a statistically prominent difference between the five groups: periodontal health,gingivitis,mild periodontitis,moderate periodontitis,and severe periodontitis(P<0.05);Among them,there weren’t prominent difference between the two groups: the group with mild periodontitis and the group with gingivitis,the group with gingivitis and the periodontal health group(P>0.05);while the differences between the other groups were statistically prominent(P<0.05).The serum ST2 level in the severe periodontitis group(18.3645 ± 1.19071)was higher than that in the moderate periodontitis group(17.1635 ± 1.32793),the mild periodontitis group(16.0556 ± 1.16686),the gingivitis group(15.5350 ± 1.22315),and the periodontal health group(14.8150 ± 0.84242).The serum UA level in the severe periodontitis group(359.2250 ± 19.54002)was higher than that in the moderate periodontitis group(333.5509 ± 19.44249),the mild periodontitis group(313.1872 ± 15.99777),the gingivitis group(306.5533 ± 14.15119),and the periodontal health group(298.9994 ± 13.02351).4.Correlation analysis of periodontal status with serum ST2 and serum UA levels in research objectsThere is a positive correlation between serum ST2 levels and PD(r=0.618,P<0.001),CAL(r=0.640,P<0.001),BI(r=0.459,P<0.001),CI(r=0.363,P<0.001),but there is no statistical significance in correlation analysis with PLI(r=0.162,P=0.103),DI(r=0.143,P=0.152);there is a positive correlation between serum UA levels and PD(r=0.654,P<0.001),CAL(r=0.690,P<0.001),BI(r=0.458,P<0.001),CI(r=0.322,P=0.001),but there is no statistical significance in correlation analysis with PLI(r=0.162,P=0.103),DI(r=0.143,P=0.152).5.Analysis of binary logistic regression and receiver operator characteristic curve(ROC curve)between serum ST2 level,serum UA level,and chronic periodontitisThe area under the serum ST2 curve(AUC)is 0.854,the Yoden index is 0.603,the sensitivity is 0.746,and the specificity is 0.857;The AUC of serum UA is 0.861,the Yoden index is 0.600,the sensitivity is 0.657,and the specificity is 0.943.Conclusion:The levels of serum ST2 and serum UA are closely related to the occurrence,severity,and prognosis of coronary heart disease.The results of this experiment show that there is a correlation between serum ST2 and serum UA levels in patients with coronary heart disease and periodontal status,and a positive correlation with depth of exploration,loss of attachment,bleeding index,and tartar index.The higher the degree of periodontitis,the higher the levels of serum ST2 and serum UA,indicating a certain correlation between periodontitis and coronary heart disease,provided clinical basis for the correlation research between them;the levels of serum ST2 and serum UA have certain reference value in the risk assessment of periodontitis in patients with coronary heart disease,providing new clinical ideas for the diagnosis,treatment methods,and prognosis of coronary heart disease and periodontitis.
