Construction And Application Of Nursing Standard Operating Procedure Framework In Post-anesthesia Care Unit

Objective:(1)To establish the framework of nursing standard operating procedure in post-anesthesia care unit;(2)Based on the framework of nursing standard operating procedure in post-anesthesia care unit,some frameworks were selected for empirical research to evaluate the clinical effect.Methods:(1)Literature research: By systematically consulting Chinese and foreign databases and professional association websites,relevant contents were extracted to provide theoretical basis for the construction of nursing standard operating procedure framework in post-anesthesia care unit;(2)Semi-structured interview method: Five nurses were interviewed to provide a practical basis for the framework of nursing standard operating procedure in post-anesthesia care unit.(3)Modified Delphi technique: Three rounds of expert consultation were conducted among 15 experts from 11 regions in China,and a scientific,reliable and systematic framework of nursing standard operating procedure in post-anesthesia care unit was initially constructed,and the reliability and validity were tested.(4)Quasi-experimental study: According to the order of admission,70 patients who were admitted to the post-anesthesia care unit after surgery in October 2021 were selected as the control group,and 70 patients who were admitted to the post-anesthesia care unit after surgery in April 2022 were selected as the experimental group.Patients in the control group were transferred to the post-anesthesia care unit through the conventional procedure,and patients in the experimental group were transferred to the post-anesthesia care unit through the newly constructed standard operating procedure.To compare two groups of patients into the post-anesthesia care unit always time-consuming,patients into the incidence of adverse events during the post-anesthesia care unit,patient transfer quality score.Results:(1)Through literature review and semi-structured interview method,the draft framework of nursing standard operating procedure in post-anesthesia care unit was drawn up,including 6 first-level indicators,19 second-level indicators and 80 third-level indicators.(2)After three consultations,the positive values of the experts in the three consultations were 88.24%,100.00%,and 100.00% respectively;The expert authority index was 0.770,0.794,0.817;The Kendall’s harmony index was 0.210,0.248,0.268,and the significance results were all P<0.001,which was statistically significant.The framework of nursing standard operating procedure in post-anesthesia care unit including6 indicators,22 second-level indicators and 97 third-level indicators was preliminarily constructed;(3)The Cronbach’α of each dimension of the nursing standard operating procedure framework in the post-anesthesia care unit was 0.828-0.938,the content validity index of each item was 0.865-1.000,and the average content validity of the scale was 0.986,which showed good reliability and validity.(4)The total transfer time of experimental group was(4.41±0.36)min,and that of control group was(5.42 ± 0.57)min,the difference between the two groups was statistically significant(P<0.001).(5)The incidence of adverse events during transfer to the post-anesthesia care unit was 4.29% in experimental group and 25.71% in control group,and the difference between the two groups was statistically significant(P<0.001).(6)The scores of handover quality between patients with anesthetists and itinerant nurses in experimental group were(4.37±0.64)points and(4.54±0.56)points,which were significantly higher than those in control group(2.13±0.74)points and(1.99±0.65)points.Conclusion:(1)Through literature review,semi-structured interview and modified Delphi technology,the nursing standard operating procedure framework of post-anesthesia care unit is scientific and reliable,which can guide nurses in post-anesthesia care unit to carry out clinical nursing work with high quality and high standard.(2)The newly constructed standard operating procedure of patient transfer in post-anesthesia care unit can improve the efficiency of clinical nursing work,reduce the occurrence of nursing errors,and ensure the continuity and homogeneity of nursing work.(3)The successful practice of standard operating procedure in the post-anesthesia care unit has confirmed its good clinical applicability and high efficiency.
