The occurrence of Chemotherapy induced phlebitis(CIP)not only increases the pain of intravenous infusion,but also often affects the orderly process of diagnosis,treatment and nursing,so the prevention and treatment of CIP in clinical intravenous infusion is very necessary.External application of potato,as the most commonly used method of external application of plants to treat CIP is also widely used because of its economy,simplicity and effectiveness,but its mechanism is not completely clear.(1)In this study,the efficacy of external application of fresh potato chips and 50%magnesium sulfate wet compress in the prevention and treatment of chemotherapeutic phlebitis was systematically evaluated.According to the set research objects and inclusion and exclusion criteria,the literature collection was divided into two time stages,and two meta-analysis studies were carried out in order to provide reference for clinical application.According to the first stage of PICO principle of meta-analysis,the time of literature retrieval: the database would be built until December 2017.participants: patients with chemotherapeutic phlebitis and patients with intravenous infusion of high concentration or hyperosmotic,strong irritant solution.The observation group was treated with external application of potato chips,and the experimental group was treated with wet application of magnesium sulfate.The first phase of the study,involving 964 patients,including potato chip group(n = 486)and magnesium sulfate group(n = 478)in meta-analysis.The random effect model was used to test the total effect(P < 0.01).The results of meta-analysis of the total effective rate showed that the external application of potato chips was better than the wet application of magnesium sulfate in the prevention and treatment of phlebitis,and the results were statistically significant.The second stage,the time range of literature retrieval: the database was built to January 2022.participants: patients with phlebitis.The limited observation group was the patients with external application of fresh potato chips,and the experimental group was the patients with 50% magnesium sulfate wet application.Non-randomized controlled studies or more than two control groups were excluded,and the use of intravenous infusion was not limited,as well as the patient’s age,type of disease,sex and external application observation days.In the second phase of the study,there were 1557 clinical patients.There were 808 cases in fresh potato slice group and 749 cases in 50%magnesium sulfate group.The results of meta-analysis showed that the effective rate of external application of fresh potato chips was better than that of 50% magnesium sulfate wet compress group,and the results were statistically significant.(2)To investigate the protective effect of α-solanine on vascular endothelial cell inflammation induced by 5-fluorouracil.In the process of meta-analysis,by collecting and reading a large number of domestic and foreign literature,it was found that potato glycoside alkaloid contained in potato had a good effect of eliminating local inflammation,relieving pain,anti-histamine and anti-allergic reaction.In animal experimental studies,it could obviously inhibit and alleviate the edema and inflammation of experimental animals,and had pharmacological activities such as anti-pathogenic microorganisms,enhancing immunity,anti-inflammation and analgesia.The effective components of potato glycoside alkaloids were analyzed,in which α-solanine and α-chaconine accounted for more than 90% of the total content,which was the main form.Among them,α-solanine has a wide range of biological activities,such as anti-tumor,anti-microbial and anti-parasite,but its potential drug activity and synergistic mechanism are not deep enough,so it has a certain research and application prospect.In the experimental study,human umbilical vein endothelial cells(HUVECs)were divided into blank control group,5-fluorouracil group,α-solanine group and5-fluorouracil + α-solanine group.Cells in 5-fluorouracil group were stimulated with5-fluorouracil at final concentration of 10 mmol/L for 24 hours,α-solanine group withα-solanine at final concentration of 5 μmol/L for 24 hours,and 5-fluorouracil + α-solanine group with 5-fluorouracil at final concentration of 10 mmol/L and α-solanine at final concentration of 5 μmol/L for 24 hours.After 24 hours,the expression of GSDME and GSDMD in blank control group and 5-fluorouracil group was detected by immunofluorescence staining.After 24 hours,the cells of each group were collected,and the expression of GSDME,GSDMD and IL-1β was detected by Western blotting and Reversetranscription-PCR.The results of immunofluorescence staining showed that 5-fluorouracil stimulation significantly increased the amount of GSDME in the cytoplasm of human umbilical vein endothelial cells,but there was no significant change in GSDMD.The results of RT-PCR and Western Blot experiments showed that 5-fluorouracil stimulation significantly increased the expression of GSDME and IL-1β in human umbilical vein endothelial cells,but there was no significant change in the expression of GSDMD.α-solanine could inhibit the expression of GSDME and IL-1β induced by 5-fluorouracil stimulation.To sum up,the results of meta-analysis in this study suggest that external application of fresh potato chips is effective in the prevention and treatment of chemotherapeutic phlebitis,and the effect is better than 50% magnesium sulfate wet compress.In the experimental study,5-fluorouracil can induce vascular endothelial cell inflammatory response,which is characterized by GSDME rather than GSDMD-mediated Pyrolysis,while α-solanine can inhibit the GSDME and IL-1β signal pathway induced by5-fluorouracil stimulation and reduce cell inflammatory response.The medicinal efficacy of α-solanine is being further studied in the field of medicine.Deep excavation and external application of fresh potato chips for the prevention and treatment of chemotherapeutic phlebitis can not only reduce the pain of patients,but also reduce medical expenses,which is the direction of traditional Chinese medicine in the prevention and treatment of phlebitis.
