Objective:Improve the diagnostic efficiency and accuracy of plasma cell mastitis and granulomatous mastitis with more refined and effective observation indicators,so that clinicians can give timely and symptomatic treatment to the patient’s disease type and avoid treatment delays due to misdiagnosis.On this basis,the overall treatment effect and medical satisfaction of patients suffering from the above two breast diseases are improved.Background:The medical community defines plasma cell mastitis and granulomatous mastitis as benign non-specific inflammatory diseases,both of which are more likely to occur in women during non-lactation and non-pregnancy;It is a clinically common adult non-lactation mastitis,the specific symptoms of which are local breast tissue masses and recurrent breast abscesses.Because the clinical manifestations and imaging indications of the above two types of breast inflammation are similar,it is very easy to misdiagnose each other in clinical diagnosis.Scholars in the medical community have been working on the study of distinguishing between plasma cell mastitis and granulomatous mastitis in various aspects and improving the accuracy of diagnosis.At present,the diagnosis of the two is mainly based on the comprehensive analysis of clinical manifestations,histopathology and auxiliary examinations,of which the results of histopathological examination are "gold standards";Ultrasonography is the most common auxiliary examination method,the Ultrasonic BI-RADS classification in the United States provides standardized description terms and risk assessment classification methods for describing various common breast diseases,in addition,ultrasonography has the characteristics and advantages of simple and convenient operation,non-invasive and effective,and has good clinical application value.However,the differential diagnosis of granulomatous mastitis and breast cancer,as well as the differential diagnostic criteria and observation indicators between plasma cell mastitis,granulomatous mastitis and other types of mastitis,need to be further accurate and refined,so as to improve the diagnostic accuracy of different types of mastitis.Methods:From February 2019 to February 2021,100 patients with mastitis who went to the Functional Examination Department of Haici Hospital for examination were selected,all of whom were female in the age range of 30 to 40 years,with an average age of 33.31(years)(standard deviation of 2.149 years),body mass index range of 19.8 to 27.3(kg/m2),and average body mass index of 22.614(kg/m2)(standard deviation of 1.66 kg/m2).In this study,the 100 patients collected were divided into two groups of 50 people in each group,one group was diagnosed with plasma cell mastitis(PCM group)after pathological examination,and the other group of patients were all diagnosed with granulomatous mastitis(GM group).After grouping,the imaging data of the two groups of patients admitted to the hospital were collected,and the ultrasound morphology,edge finishing,edge blur,edge angle,edge microlobeing,edge burr,internal echo,peripheral fat.BI-RADS were compared with the high-frequency chromatic ultrasound imaging examination of the two groups of patients.Whether the edges can detect the echo of the inner membrane and the thickness of the inner membrane and analyze the similarities and differences.Through the comparison of various indicators,it is pointed out that the ultrasonic observation indicators of the two diseases are effectively distinguished and the diagnostic accuracy of the two mastitis diseases is calculated by high-frequency color ultrasound diagnosis;On this basis,the differential diagnostic value and significance of high-frequency color ultrasound imaging characteristics for plasma cell mastitis and granulomatous mastitis were analyzed.Results:There were no significant differences in ultrasound morphology,edge finishing,edge blur,edge angularity,edge microlobeing,edge burrs,internal echo,peripheral fat,and BI-RADS classification in the pcm and GM groups,and there was no statistical significance(P>0.05).The two indicators of intimal echo and intimal thickness were remarkably different between the two groups.After analysising the high-frequency color ultrasound imaging data,the plasma cell mastitis color ultrasound image can be seen to show obvious duct dilation,that is,the intimal structure of the catheter wall(bilayer structure at the edge),the thickness is about 0.5 mm,and the inner membrane has an echo,while the marginal part of granulomatous mastitis has only a monolayer structure and no endometrial echo.Conclusions:After comparative analysis and data statistics of the imaging data of the two groups of patients,it can be found that whether the inflammatory edge can probe the intimal echo and the thickness of the intimal membrane(about 0.5mm)is an important basis for distinguishing plasmacytic mastitis and granulomatous mastitis,and the identification accuracy rate is as high as 92%.Differential diagnosis of the two diseases with the above indicators helps physicians to carry out follow-up treatment for the patient’s specific type of mastitis as soon as possible and avoid delays;At the same time,it can improve the treatment effect and the patient’s satisfaction with the treatment,which is worth promoting and applying.
