Objective To explore the correlation between the number of high signal of blood vessels in MRI fluid-attenuated inversion recovery vascular hyperintensity(FVH)and the parameters of CT perfusion(CTP)at the peripheral area of cerebral infarction in patients with cerebral infarction,so as to explore the value of FVH sign in collateral compensation assessment of cerebral ischemic stroke,and to provide decision-making basis for clinical treatment of patients with cerebral infarction.Methods Analyzedthe clinical and imaging data of 75 patients with acute cerebral infarction admitted from January 2018 to January 2020 retrospectively.MRI scanning sequence includes T1 WI,T2WI,T2 plain,DWI,MRA.Two neuroimaging physicians above the subtropical high level read the MRI images of all patients for diagnosis and analysis,determine whether there is FVH,and measure FVH for patients with positive FVH.The statistical method for the number of FVH signs is as follows:Record each spot,strip or serpentine high signal that meets the FVH standard in T2-FLAIR image as a FVH count,count the number of FVHs on each layer of the image on the affected side of the patient,add the number of FVHs on each layer of the image to get the total number of FVHs of the patient as the number of FVHs of the patient.CTP examination collected parameters including Cerebral Blood Volume(CBV)、Cerebral Blood Flow(CBF)、Mean Transit Time(MTT)、Time to Peak(TTP).Manually delineated the area around the cerebral infarction(low perfusion area around the core area of cerebral infarction,that is,the area where MTT and TTP were prolonged,but CBV remained unchanged or increased).Divide each parameter value of the affected side by the value of the healthy side to obtain the change ratio of the perfusion value of the affected side.According to the presence or absence of FVH signs,the patients were divided into FVH positive group and FVH negative group.The clinical basic data and CTP parameters of the two groups were analyzed by T test,U test and chi-square test respectively,and the correlation between the number of FVH and CTP parameters of the FVH positive group was analyzed by Spearman correlation coefficient.Results There were 75 patients with acute cerebral infarction who met the inclusion criteria,including 54 patients in the FVH positive group and 21 patients in the FVH negative group.(1)The common clinical risk factors of cerebrovascular disease have no obvious influence on the formation of FVH.(2)There were differences in r MTT,r TTP and r CBV in periinfarct area between FVH positive group and FVH negative group(P<0.05);There was no significant difference in r CBF between the two groups(P>0.05).(3)The number of FVH was positively correlated with r TTP、r MTT、r CBV(P<0.05),but not significantly correlated with r CBF(P>0.05).Conclusions In patients with cerebral infarction,the perfusion parameters in the periinfarct area of patients with FVH sign were significantly better than those without FVH sign,and the higher the FVH score(the more the number),the better the blood perfusion in the periinfarct area,suggesting that FVH is an effective means to evaluate the degree of collateral compensation of cerebral ischemia. |
