| ObjectiveThe aims of this study are introducing the Stressor Scale of Emergency Nurse(SSEN),translating it into Chinese version,and testing the psychometric properties.The Chinese version of the scale was applied to evaluate the current situlation of occupational stressor level of clinical nurses in emergency departments and analyze its influencing factor,explore the relationship between work engagement and occupational stress,and introduce individual resistance as the mediating variable to test the mediating effect.MethodsPart 1:The scale was authorized by the original author of SSEN,and sinicized according to the modified Brislin translation model.The contents of the Chinese version completed in sinicization were adjusted cross-cultural by Delphi method and pre-test,and the content validity of the scale after sinicization was evaluated by correspondence with experts.Part 2:The Chinese version of the Stressor Scale of emergency nurse was used to conduct an electronic questionnaire survey among 401 emergency department nurses in four third-class A hospitals in Qingdao.The obtained data were verified by Epidata,Excel,SPSS and Amos statistical software for the item analysis(extreme value method,correlation coefficient method),calibration correlation validity,structural validity(exploratory factor analysis,confirmatory factor analysis),reliability(internal consistency,split-half reliability,retest reliability).Part 3:According to the Job Demand-Control-Support theory,517 nurses in emergency departments were investigated by using the general data questionnaire,the Chinese version of the Stressor Scale of Emergency Nurse,Utrecht Work Engagement Scale and the Resilience Structural scale of medical staff.SPSS was used to analyze the general information of the research object,the current situation of occupational stress of emergency nurses,and the significant influencing factors of occupational stress.The Process plug-in was used to analyze the mediating effect of individual resilience between occupational stress and work engagement of emergency nurses.ResultsPart 1:After cross-cultural debugging and pre-investigation of the scale after sinicization,the Chinese version of SSEN consisting of 27 items and 4 dimensions(acute patient care,patient and family support,technical and organizational support,work environment and interpersonal relationship).The I-CVI were 0.89~1.00,S-CVI/UA=0.81,S-CVI/AVE=0.980.Part 2:The critical rations of 27 items ranged from 8.385 to 17.364(P<0.001),and the correlation coefficient between the scores of each item and the total score of the scale was0.588 to 0.836(P<0.001).Four common factors were extracted by exploratory factor analysis,and the variance contribution rate was 78.463%.Confirmatory factor analysis of structural equation model as amended,χ~2/df=2.280,CFI=0.933,TLI=0.924,IFI=0.933,RMSEA=0.079;The total Cronbach’sαcoefficient of the Chinese version of SSEN was0.971,for the dimensions ranged in 0.940~0.955.The total split-half reliability was 0.877,and the split-half reliability of each dimension was 0.889~0.909.The total re-test reliability is 0.863,and the re-test reliability of each dimension was 0.624~0.713.Part 3:The total score of nurses’occupational stress in emergency department was78.70±25.64,and the average score of each dimension from highest to lowest was Patient and family support(3.12±1.07),emergency patient care(2.99±1.06),technical and organizational support(2.77±1.10),work environment and interpersonal relationship(2.72±1.09);The results of multiple linear regression showed that education level,technical title and working years had significant effects on the occupational stress of emergency nurses,which accounted for 15.5%variation rate of the occupatioanl stress of emergency nurses.The mediation analysis showed that the total effect was significant,and the indirect effect of individual resistance was significant.The direct effect and indirect effect accounted for 95.86%and 4.14%of the total effect,respectively.Three significant influence discovered in this study were as follows:"work engagement→occupational stress","work engagement→individual resilience"and"individual resilience→occupational stress".Conclusions(1)The Chinese version of Stressor Scale of Emergency Nurse has 4 dimensions and 27items,which shows good reliability and validity in the emergency nurses.It is suitable for the current situation of nursing clinical work in emergency departments in China;(2)The occupatioal stress of emergency nurses in this study was at a middle level,and the main stressors were patient and family support and emergency patient care;Education level,technical title and working years were the main factors affecting the occupational stress of emergency nurses.Work engagement and individual resilience have negative effects on occupational stress,and individual resilience plays a partial mediating role between work engagement and occupational stress.Clinical nursing managers should pay attention to the occupational stress of emergency nurses,formulate coping measures according stressors,and reduce their occupational stress level through multi-angle intervention. |
