Objective:To investigate the clinical and immunohistochemical data of Ta and T1 stage non-muscular invasive bladder cancer,and to analyze the influence factors of CK20and Ki67 on postoperative recurrence.Methods:The clinical data and postoperative follow-up data of 94 patients with TURBT and Ta and T1 stage NMIBC treated in Shanxi Bethune Hospital from January 2017to January 2021 were analyzed retrospectively.According to the relapse,the patients were divided into two groups:relapse group(n=24)and no relapse group(n=70).The differences of sex,age,smoking history,tumor grade,risk group,tumor size,number of tumors,expression of CK20 and Ki67 between the two groups were compared.χ~2test was used for comparison between groups.Multi-factor analysis using COX regression model.The survival curve was drawn by Kaplan-Meier method,and the survival difference was tested by Log-rank test.The influencing factors of CK20 and Ki67 on the relapse of Ta and T1 non-muscular invasive bladder cancer were analyzed.Results:The mean survival time without recurrence was(21.26±1.18)months.There were no relapse in 70 cases and relapse in 24 cases.CK20 was positive in 56 cases,of which 17 cases relapsed,29 cases were Ki67 positive,and 16 cases recurred.Univariate analysis showed that tumor grade,risk grouping,tumor size,tumor number,Ki67 were correlated with recurrence in patients with Ta and T1 NMIBC.Then multivariate analysis showed that non-low risk group(0.013)and the positive expression of Ki67(0.000)were independent risk factors for postoperative recurrence in patients with Ta and T1 NMIBC.Conclusion:1.High-grade tumor,non-low-risk group,tumor≥3cm,multiple tumors and positive Ki67 were the risk factors of recurrence in patients with Ta and T1 NMIBC.2.Ki67 positive and non-low risk groups are independent influencing factors of recurrence in patients with Ta and T1 NMIBC.The combination of basic clinical data and biological indexes has certain clinical value in predicting the risk of recurrence in patients with Ta and T1 NMIBC,which should be paid attention to in clinical intervention. |
