Objective:Taking inpatients with diabetic foot as the research object,we constructed a positive psychological intervention program under PERMA model in order to alleviate patients’ negative emotions of depression and anxiety and somatic symptoms of insomnia and pain,and initially applied it to patients,verified the clinical effect of the program through practical application,and provided a more scientific,systematic and practical new program and new ideas for the psychological care of medical and nursing staff.Methods:1.Construction of the program:(1)Through literature review and analysis,with positive psychology as the theoretical basis,PERMA model as the framework,and health education of diabetic foot combined with education of diabetic specialist disease as the root,we provided theoretical support for the development of psychosomatic intervention program of diabetic foot;(2)Using expert meeting method,in order to ensure the scientificity,feasibility and effectiveness of the intervention program of psychosomatic symptoms of diabetic foot.Experts from mental health center,endocrinology,vascular surgery,plastic and burn surgery and other related fields were invited to hold an expert group meeting to discuss and revise the content of the initially constructed intervention program;(3)Pre-experimentation was conducted to select 10 clinical patients who met the inclusion and exclusion criteria for pre-experimentation,summarize the force majeure factors and feedback from patients’ opinions that occurred during the experiment,and revise and form the final intervention protocol.2.implementation of the program: 62 inpatients with diabetic foot who met the inclusion and exclusion criteria in the Department of Plastic and Burn Surgery of a tertiary care hospital in Shanxi Province were selected as the study subjects,and the study subjects were divided into two groups according to the order of admission and the random number method generated by SPSS,one group was the control group(n=31)and the other group was the experimental group(n=31),the interventions in the control group were diabetic foot specialist care and routine The interventions in the control group were specialized diabetic foot care and conventional health education,and the interventions in the experimental group were specialized diabetic foot care and positive psychological intervention based on the PERMA model.The evaluation tools were Self-Rating Depression Scale(SDS),Self-Rating Anxiety Scale(SAS),Athens Insomnia Scale(AIS),and Numerical Rating Scale(NRS).Results:1.Construction of the program: a literature review was conducted to provide the theoretical basis for the initial development of the intervention program;the scientificity and feasibility of the program were revised after the expert group meeting and pre-experiment,and the PERMA model positive psychological intervention program with seven different themes and different exercise contents was finally developed.2.Implementation of the program:(1)the general information of the test and control groups were compared,and the differences were not statistically significant(P>0.05);(2)before the intervention,the SDS scores,SAS scores,AIS scores and NRS scores of the two groups were not statistically significant(P>0.05),and the two groups were balanced and comparable;(3)after the intervention and 2 weeks after the intervention,the SDS,SAS,and AIS scores were lower than those of the control group,and the differences were statistically significant(P < 0.05).The differences were statistically significant(P < 0.05)for SAS,SDS,and AIS scores under the group effect and time effect between groups,and there was an interaction effect of time factor and grouping(P < 0.05);(4)The NRS scores in the test group were lower than those in the control group after the intervention,and the differences were statistically significant(P < 0.05),and the NRS scores in the test group were lower than those in the control group 2 weeks after the intervention,and the differences were not The difference was not statistically significant(P > 0.05).The differences in NRS scores at different time points between the groups were statistically significant(P < 0.05).Patients in both groups were in a stable condition during the intervention phase,and no adverse effects were observed.Conclusion:1.At the stage of intervention program construction,this study used scientific research methods of literature review,expert group meeting and pre-experiment to construct a set of positive psychological intervention program for psychosomatic symptoms of diabetic foot patients with PERMA model as a framework in a more systematic way.2.Through preliminary clinical application,this study demonstrated that positive psychological intervention based on the PERMA model can reduce the negative emotions of depression and anxiety and the somatic symptoms of insomnia and pain in diabetic foot patients.This care program can improve the quality of life of diabetic foot patients,and provides new ideas and methods for clinical health care professionals to carry out psychological care,which has certain reference value. |
