Objective:A retrospective study was conducted to collect the left ventricular function level in maintenance hemodialysis(MHD)patients,and to investigate the correlation between blood magnesium level and left ventricular insufficiency in maintenance hemodialysis(MHD)patients.Methods:A total of 133 hemodialysis patients with complete data who were treated in the Blood Purification Center of the Department of Nephrology,the First Hospital of Shanxi Medical University from October 2021 to June 2022 were selected to meet the diagnostic criteria of left cardiac insufficiency with both LVEF < 50% and FS < 25%.Patients were divided into left ventricular dysfunction group(n=23)and non-left ventricular dysfunction group(n=110).The general data and clinical indicators of patients were collected and sorted,and the relationship between the left heart function level of patients in each group and serum magnesium ion and other related factors was analyzed by statistical methods.SPSS22.0 statistical software was used for statistical analysis of the data,and P < 0.05 was considered statistically significant.Results:(1)General information of 133 MHD patients: the average age of enrolled patients was 46.52±12.00 years old,including 76 males,accounting for 57.1% of the total,and 57 females,accounting for 42.9% of the total.Among the primary diseases,60 cases of chronic glomerulonephritis,accounting for 45.1% of the total,43 cases of diabetic nephropathy,accounting for 32.3% of the total,12 cases of hypertensive kidney damage,accounting for 9% of the total,and 18 cases of other diseases,accounting for 13.6% of the total.(2)There were statistically significant differences in age,dialysis age,calcium(Ca),phosphorus(P),magnesium(Mg),calcium and phosphorus product,left ventricular end-diastolic internal diameter(LVEDD),left ventricular end-diastolic internal diameter(LVESD),left ventricular end-diastolic volume(LVEDV)and left ventricular end-systolic volume(LVESV)between 2 groups(P < 0.05)).(3)Spearman correlation analysis showed that left heart function was negatively correlated with age(r=-0.371,P < 0.001),dialysis age(r=-0.177,P=0.042),Ca(r=-0.225,P=0.009),and positively correlated with Mg(r=0.199,P=0.022).Mg was negatively correlated with LVEDD(r=-0.176,P=0.043),LVESD(r=-0.187,P=0.031)and LVESV(r=-0.189,P=0.03).(4)Binary Logistic regression analysis showed that age,dialysis age and Ca were risk factors for left ventricular insufficiency in MHD patients,and Mg was protective factor for left ventricular insufficiency in MHD patients.(5)The patients were divided into two groups based on the median Mg.The age and Ca level of the low Mg group were higher than that of the high Mg group(P<0.05),but there was no statistical significance in dialysis age between the two groups(P>0.05).Binary Logistic regression analysis showed that age and Ca level were influencing factors of Mg level in MHD patients(P < 0.05).Conclusions:There is a high incidence of left ventricular insufficiency in MHD patients,and an increase in blood magnesium level within a certain range can reduce the occurrence of left ventricular insufficiency in MHD patients.Magnesium level may affect left ventricular function by affecting left ventricular end-diastolic diameter,left ventricular end-systolic diameter and volume.Blood magnesium level in MHD patients may be affected by age and blood calcium,which may further affect left heart function by affecting blood magnesium.
