| Objective:To evaluate the diagnostic value of dermoscopy in actinic keratosis(AK).The correlation between dermoscopic features and histopathologic findings of AK was also discussed.Methods:A total of 37 patients clinically diagnosed as AK were collected from the dermatology clinic of the First Hospital of Shanxi Medical University between June 2020 and October 2022.Dermoscopic and histopathologic diagnosis and grading were performed for their lesions successively.Histopathologic diagnosis was used as the gold standard to calculate the sensitivity,specificity,agreement rate,Youden’s index,Kappa value,positive predictive value and negative predictive value of dermoscopy in the diagnosis of AK.Kappa test was used to discuss the consistency of dermoscopy and histopathology in grading of AK.Chi-square test or Fisher’s exact test was used to discuss the correlation between dermoscopic features and histopathologic types of AK.Results:The sensitivity,specificity,agreement rate,Youden’s index,Kappa value,positive predictive value and negative predictive value of dermoscopy in the diagnosis of AK was85.19%,80.00%,83.78%,65.19%,0.613(P<0.05),92.00% and 66.67%,respectively.Overall dermoscopic and histopathological gradings were consistent in 25 cases,and the coincidence rate was 67.57%.Dermoscopy and histopathology in grading of AK showed a moderate consistency(P<0.05).The difference between four dermoscopic groups was not statistically significant(P>0.05).Conclusion:The dermoscopic method evaluated in this paper has a high diagnostic efficiency and the application of dermoscopy can improve the clinical diagnostic accuracy of AK.Dermoscopy and histopathology showed a moderate consistency in the grading diagnosis of AK.Correlation between dermoscopic features and histopathologic types of AK can not be considered. |
