ObjectiveThis article aims to construct a nursing intervention plan for patients with kinetic phobia after total knee arthroplasty,and evaluate its clinical application effect.MethodsThrough literature review,an expert consultation form for nursing intervention plan for patients with kinesiology after total knee arthroplasty was compiled;20 experts were selected by purpose sampling for two rounds of consultation,and the nursing intervention plan was evaluated by combining expert opinions and index threshold screening method.The patients with acrophobia after total knee arthroplasty were selected from March2021 to March 2022,who were hospitalized in the joint surgery ward of a third-class A general hospital in Qingdao.According to the different admission date,they were randomly divided into control group(N=80)and intervention group(N=80).The control group was given routine nursing care for total knee arthroplasty,and the intervention group was given individualized exercise+rehabilitation+psychological triple panic intervention program on the basis of nursing care in the control group.The general information,degree of fear and pain,knee function,compliance with functional exercise,length of stay and expenses of the two groups were compared before and after the intervention.Results1.Through 2 rounds of Delphi expert consultation,a nursing intervention plan for patients with motion phobia after total knee arthroplasty was finally constructed,consisting of 2 level 1 items,6 level 2 items,and 24 level 3 items.The authority levels of the results of the two rounds of expert correspondence inquiries were both greater than 0.7,and the coordination levels of the two rounds of expert opinions were 0.241 and 0.305,respectively.2.This study included 160 patients with kinetic disorder after total knee arthroplasty who were hospitalized in the joint surgery ward of a tertiary A hospital in Qingdao from March 2021 to March 2022.According to different admission time,they were divided into intervention group(N=80)and control group(N=80).There was no significant difference between the intervention group and the control group in age,gender,time of pain,education level,marital status,average monthly income,whether they lived alone,history of smoking and drinking,and whether they had medical insurance or not(P>0.05).3.There was no significant difference in the scores of kinetophobia between the intervention group and the control group(P>0.05).Compared with the score of kinetic disorder at discharge,1 month and 3 months after operation,the control group was significantly higher than the intervention group(P<0.05).4.There was no significant difference in the knee joint function score between the intervention group and the control group(P>0.05).Compared with the knee joint function scores at discharge,1 month and 3 months after operation,the scores of the control group were significantly lower than those of the intervention group(P<0.001).5.There was no significant difference in the functional exercise compliance score between the intervention group and the control group at admission(P>0.05).Comparing the scores of functional exercise compliance at discharge,1 month and 3 months after operation,the control group was significantly lower than the intervention group(P<0.05).6.There was no significant difference in the NRS score between the intervention group and the control group at admission(P>0.05).Compared with the NRS score at discharge,1month and 3 months after operation,the intervention group was significantly lower than the control group(P<0.05).7.The average length of hospitalization and hospitalization expenses in the intervention group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).ConclusionIn this study,a nursing intervention program for patients with kinesiphobia after total knee arthroplasty was constructed by means of expert consultation.The experts consulted by this program have a high degree of authority and the results are reliable.The constructed nursing intervention plan for patients with phobia after total knee arthroplasty can significantly improve the patient’s phobia score and knee function score,while reducing postoperative pain and improving compliance with functional exercise.In addition,the average length of hospitalization and hospitalization expenses of patients in the intervention group were also significantly lower than those in the control group.The results show that the nursing intervention plan constructed in this study has a good application effect in the nursing care of patients with kinetic phobia after total knee arthroplasty,which can provide a reference for clinical nursing practice. |
