In recent years,with the continuous increase of people’s demand for rational drug use and the gradual deepening of the reform of the medical and health system,pharmaceutical care has been rapidly developed,the service scope has been gradually expanded,and the service connotation has become richer.Many pharmacists use their own professional means to provide medication education,medication consultation and other services for patients.Pharmaceutical clinics in medical institutions are also gradually opened,but most of them are free services or there is no clear collection and payment system,so the value of pharmaceutical services has not been reflected.At present,the National Medical Insurance Administration has worked with relevant departments to sort out pharmaceutical services with clear patient needs,specific service contents and objects and clear technical routes,and studied and improved pharmaceutical medical services price items to support the healthy and orderly development of pharmaceutical services.Under the background of medical reform,it is an inevitable trend to increase pharmaceutical service fees by improving the level of pharmaceutical care.Scientific and reasonable pharmaceutical care pricing and payment management system can improve the framework of pharmaceutical care system,promote the sustainable development of pharmaceutical care,further realize the transformation of pharmaceutical care mode from "drug-centered" to "patient-centered",and reflect the value of pharmacists.This paper takes pharmaceutical outpatient service in Shanghai as the research background,focuses on how to establish the charging mode and standard of pharmaceutical outpatient service in Shanghai,comprehensively uses literature research method,expert interview method,standardized value model,and summarizes the research on the pricing and charging of pharmaceutical outpatient service at home and abroad,and combines the actual situation and key concerns of outpatient service in Shanghai.This paper actively explored and analyzed the charging mode and pricing standard of pharmaceutical outpatient service in Shanghai,and finally provided pilot samples and policy suggestions for promoting the collection of pharmaceutical service fees nationwide.This study firstly expounds the development of pharmaceutical care,cost measurement and pricing methods,payment standards and payment methods,regulatory assessment and supporting measures from the aspects of the research background,research purpose,research significance and domestic and foreign literature review,and draws on the empirical measures of pharmaceutical care pricing and payment from typical countries and regions.Conceptually,the research problems were described in detail by distinguishing between pharmaceutical service fees and pharmaceutical service fees,and the similarities and differences between different modes of pharmaceutical service fees.Theoretically,the theoretical basis of pharmaceutical outpatient service pricing and calculation in Shanghai was discussed by combining stakeholder theory and pharmaceutical care compensation theory.In terms of model and method,field investigation,expert consultation,standardized value model and comparison model are used to process and analyze the data.From these three parts,the positive role of pharmaceutical outpatient service fee in improving the safety and effectiveness of drug use,maintaining public health and promoting the improvement of medical level was firstly expounded.Secondly,through the theoretical analysis of pharmaceutical outpatient service pricing and fees,the feasibility of pharmaceutical outpatient service in Shanghai needs to be focused on to prepare for calculation.Through the construction of the price standard of pharmaceutical outpatient service items in Shanghai,the standardized value and fee standard of pharmaceutical outpatient service items in Shanghai were calculated,and the existing fee standard was referred to,so as to calculate the standardized value of pharmaceutical outpatient service items in Shanghai,and the calculation results were obtained by using the price comparison model under the existing price system.Based on the calculation results of pharmaceutical outpatient service fees in Shanghai,the constraints of pharmaceutical outpatient service fees in Shanghai and the main problems of the establishment of the collection mode were clarified.The development status,market environment and prospects of pharmaceutical outpatient service fees were further analyzed,and the constraints and main problems of pharmaceutical outpatient service fees in the current stage were clarified.Finally,according to the medical insurance pricing and payment management requirements of Shanghai Pharmaceutical Service Standard(Trial),based on the domestic and foreign pharmaceutical care pricing methods and payment strategies,as well as the experts in medical insurance,health and other fields,according to the calculation results of pharmaceutical outpatient service projects in Shanghai,the existing problems and difficulties in pharmaceutical care were found.A series of supporting policies such as pharmaceutical care project pricing,payment,supervision,assessment and adjustment were put forward to promote the high-quality development of pharmaceutical care.
